This case report describes a 34-year-old woman with recurrent right-sided catamenial pneumothorax, initially diagnosed as pneumothorax after referral for cough, painful breathing, and dyspnoea, with episodes treated by drainage, thoracoscopic talc pleurodesis, and later VATS with partial pleurectomy. Although thoracoscopy and chest CT showed no abnormalities and continuous hormonal suppression did not prevent further recurrences, later laparoscopy after the sixth episode identified peritoneal endometriosis with extensive diaphragmatic scarring and fenestrations, while pathology from earlier pleural tube samples showed no endometriosis; even after subsequent partial pleurectomy with pleurodesis, the patient had no further pneumothorax but continued thoracic discomfort. The major limitation is that it is a single case with negative findings on earlier procedures and no standardized pathway to confirm the diagnosis before recurrence. This paper is centrally about endometriosis — specifically extrapelvic thoracic (catamenial pneumothorax) endometriosis presenting as recurrent, menstrual-cycle–linked pneumothorax.
