This paper reports a rare case of intrinsic unilateral ureteral endometriosis in a 44-year-old woman who presented with abdominal pain, dysuria, and right mild hydronephrosis; pelvic ultrasound was unremarkable, but CT urography showed a distal ureter filling defect without stones. Diagnostic ureteroscopy identified a pedunculated intraluminal rounded mass in the lower third of the right ureter, which was endoscopically excised with laser and a double-J stent placed; histopathology reported normal endometrium tissue and no ureteric tissue. Follow-up ureteroscopy and serial CT imaging over up to 2 years showed no recurrence and resolution of hydronephrosis, though renal function was reduced due to longstanding asymptomatic obstruction confirmed by MAG3 scanning. As a case report, the major limitation is that findings cannot establish generalizable effectiveness or mechanisms, and the paper’s clinical course depended on this individual’s presentation and treatment.
