Pre-operative Mapping and Structured Reporting of Pelvic Endometriotic Lesions on Dynamic Ultrasound and Its Correlation on Laparoscopy Using the #Enzian Classification
Dynamic ultrasound with structured reporting accurately maps pelvic endometriosis, correlating well with laparoscopic findings for improved surgical planning.

This retrospective study evaluated how well structured dynamic ultrasonography using the IDEA protocol and an updated #ENZIAN (2021) scoring approach identifies deep infiltrating endometriosis compartments, by comparing pre-operative ultrasound findings with laparoscopic excisional surgery findings classified with #ENZIAN in 50 patients from a single multidisciplinary tertiary hospital. Ultrasound mapping was performed by one experienced radiologist and reported in a structured format, and the paper reports sensitivity and negative predictive values across peritoneal, ovarian, tubal, uterosacral, rectal, rectovaginal, adenomyosis, and ureteric lesion sites, with several compartment-specific performance estimates. The authors note key limitations including the retrospective design, single center and single radiologist/surgeon setting, and that analyses relied on concordance with laparoscopic classification without prospective evaluation. This paper is centrally about endometriosis — it uses IDEA-based dynamic ultrasound and updated #ENZIAN mapping to assess and correlate pelvic deep infiltrating endometriosis with laparoscopic findings.
