This case report and literature review describes a 46-year-old woman who developed clear cell carcinoma in an abdominal scar after a cesarean section performed 18 years earlier, presenting with a tender, ulcerated abdominal wall mass and large lobulated tumors on imaging and excision. Histology showed malignant cystic structures with hobnail-like clear cells and nuclear atypia, with residual ectopic endometrial glands/stromal cells supporting clear cell carcinoma arising from endometriosis; a key limitation emphasized by the authors is that benign endometrial tissue was found in only 64.7% of reviewed clear cell carcinoma cases and may require examining multiple sections to confirm. Across 17 analyzed clear cell carcinoma cases from scar endometriosis, most had a prior cesarean section, lesions were often initially misdiagnosed as benign masses, local excision was performed in all, adjuvant chemo- or radiotherapy was used in most, and 29.4% died of disease with some presentations involving inguinal/pelvic lymph nodes.
