This paper presents a case of a 33-year-old woman evaluated for a suspected ileocecal mass after an episode of abdominal pain, diarrhea, and hematochezia with CT findings concerning for ischemic colitis and subsequent resolution of mucosal ischemia but persistent ileocecal fullness on repeat colonoscopy; endoscopy and biopsies initially showed only lymphoid hyperplasia. Despite difficulty ruling out malignancy, laparoscopic right hemicolectomy was performed, with frozen section confirming an endometrioma and final histopathology showing an infiltrative cecal endometrial mass obliterating the appendix and involving ileal submucosa, plus endometriosis/endosalpingosis inclusions in several of 45 benign lymph nodes, supported by targeted immunohistochemistry. The authors frame this diagnostic challenge within proposed mechanisms of endometriosis (including retrograde menstruation and lymphovascular spread) but, as a single case report, does not establish incidence, causality, or generalizable outcomes.
