The study investigated the diagnostic value of clinical bimanual examination and transvaginal ultrasound (TVS) for preoperative detection of endometriosis-related lesions in 70 women with endometriosis-associated symptoms, using histologically verified findings after (at least partially) radical laparoscopic resection as the reference. Clinically, sensitivity was inadequate for several sites, especially adnexal, bladder, and intestinal/rectosigmoid involvement, whereas adding TVS substantially improved detection rates across sites including adnexa, bladder, vagina, and particularly bowel lesions. The paper reports site-specific sensitivities, specificities, and predictive values for clinical exam alone, TVS alone, and combined assessment, with combined use providing the most reliable inference of infiltrative pelvic endometriosis.
