A retrospective analysis of spectrum of presentation of adenomyosis in tertiary centre

This retrospective study (Aug 2016–Aug 2017) reviewed case records and histopathology of hysterectomy specimens from 50 patients at a tertiary center to characterize the spectrum of adenomyosis presentation and to compare the diagnostic accuracy of clinical examination and imaging. Most patients were aged 41–50 years, were multiparous, and commonly reported menstrual disturbances (56%), with dysmenorrhea and dyspareunia as frequent pain symptoms; fibroid was the most common associated pathology (38%). Imaging (ultrasound) detected adenomyosis in 40% of cases, clinical diagnosis in 36%, whereas gross specimen examination and HPE increased detection to 66% and 100% respectively, and the authors note that coexisting pathology could mask adenomyosis features and lead to missed diagnoses. This paper is centrally about endometriosis and/or adenomyosis — endometriosis is not the focus; it centers on adenomyosis’s varied clinical and diagnostic spectrum.
