A Case of Ruptured Decidualized Ovarian Endometrioma: Usefulness of Serial MRI for Determining Adequate Management

This paper reports a 39-year-old pregnant woman with a known right ovarian endometrioma who underwent MRI during pregnancy; the study goal was to determine how imaging could distinguish decidualized endometrioma tissue from malignancy and detect rupture. At 20 weeks, MRI showed a teardrop-shaped cystic lesion with mural nodules whose signal characteristics and diffusion properties were interpreted as consistent with decidualized ovarian endometrioma; the authors planned serial MRI about 6 weeks later. At 27 weeks, emergency MRI for sudden abdominal pain showed disappearance of the cyst with bloody ascites, leading to suspicion of ruptured decidualized endometrioma, and conservative observation was selected until an elective cesarean section with cystectomy at 37 weeks confirmed rupture and decidual reaction on histology without malignancy. As a single case report, the main limitation is generalizability, but it supports the utility of serial MRI findings.
