Abstract
症例は17歳女性．主訴は喀血．子宮内膜掻把術の既往があり，この6ヵ月後より月経の時期のみに喀血を繰り返し来院．CT上右肺下葉S9主体の浸潤影をみとめ，気管支鏡検査でも右B9末梢からの出血が疑われ，同部の肺子宮内膜症が疑われた．偽閉経療法で喀血は軽快したが，今後の学業復帰や就業，ホルモン剤離脱希望の為，病巣の切除による根治を目的に手術を施行した．右下葉背側周囲に褐色調の胸膜変化，軽度の炎症性癒着をみとめたが，鏡視下に下葉切除し得た．病理組織の詳細な検索で，子宮内膜腺および内膜間質を認めた．免疫組織化学検査では，内膜腺にER・PgR陽性，内膜間質にCD10・ER・PgR陽性を示した．術後経過は良好であった．術後3年現在喀血を認めていない．肺子宮内膜症は胸膜子宮内膜症の1/6と少なく，さらに組織学的に証明しえた希少な1例を経験した．また，本症における鏡視下手術の役割は大きいと考えられた．
