Analysis of Follicular Fluid Retinoids in Women Undergoing In Vitro Fertilization: Retinoic Acid Influences Embryo Quality and Is Reduced in Women With Endometriosis

This study quantified retinol and all-trans retinoic acid (ATRA) in matched peripheral plasma and follicular fluid from 79 women undergoing IVF at oocyte retrieval, then assessed embryo quality daily for 3 days until uterine transfer, including comparisons between endometriosis patients and controls. Follicular fluid showed distinct retinoid metabolite/isomer patterns versus plasma, and higher follicle size was associated with a >50% increase in follicular fluid retinol and ATRA; embryos from follicle size differences also related to producing a higher percentage of grade I embryos. Grade I embryos had higher mean follicular fluid ATRA (not retinol), and in a nested case–control analysis women with endometriosis had ~50% lower mean ATRA in both follicular fluid and plasma than controls. The paper states a caveat that embryo quality was correlated with measured retinoid levels but causality cannot be established from this observational design.
