Abstract

症例は42歳，女性．平成6年より子宮内膜症にて婦人科に通院中であった．平成18年11月頃から度々腹痛をきたすようになり，平成19年2月に月経時の腹痛が増悪したため近医を受診した．腹部単純X線にて小腸ガスを認め，腸閉塞の診断で当科紹介となった．CT・MRIにて遠位回腸に腫瘤を認め，子宮内膜症の既往もあることから回腸子宮内膜症が疑われた．絶食にて症状は軽快したが，小腸造影にて遠位回腸に狭窄を認めたため，開腹手術を行った．CTで認めた腫瘤を中心に，遠位回腸が巻きついて一塊となっていたため，腫瘤ごと回腸部分切除を行った．また，骨盤内には小水疱が散在しており，右卵巣は周囲と強固に癒着していた．標本の腫瘤はチョコレート嚢胞様であり，回腸子宮内膜症と診断した．回腸子宮内膜症によって腸閉塞をきたした症例を経験したので文献的考察を加えて報告する．
