Appendiceal Nodules in the Setting of Endometriosis Can Be Carcinoid Tumors

This paper reports a retrospective surgical series (108 patients with deep infiltrative endometriosis treated between July and December 2013) in which imaging identified appendiceal lesions, and two detailed case reports where appendiceal nodules presumed to be endometriosis were instead found to be appendiceal carcinoid/neuroendocrine tumors on histology. Across the 6 resected appendix specimens, 4 contained endometriotic lesions and 2 contained carcinoid tumors, with the authors noting that atypical imaging features (e.g., well-defined hypervascular nodules) did not reliably distinguish carcinoid from endometriosis and that their referral-center case mix likely increased the observed appendix involvement rate. The limitation is that carcinoid diagnosis could not be excluded preoperatively by imaging, and the findings come from a single referral center without a comparison group. This paper is centrally about endometriosis — it describes appendiceal nodules discovered during surgery for deep infiltrative endometriosis and shows that carcinoid tumors can be unexpectedly diagnosed in lesions presumed to be endometriosis.
