Abstract
症例は20歳，女性．約1カ月ほど前から右鼠径部膨隆を自覚するようになり当科を受診した．右鼠径部に約4cm大の境界明瞭・弾性硬の腫瘤を触知し，超音波検査および腹部造影CT検査にて右鼠径部に内部均一な嚢腫様腫瘤を認めた．大網や腸管，卵巣などの脱出は認めず，右Nuck管水腫と診断し手術を施行した．腫瘤は一部子宮円索と強固に癒着しており，この癒着部にて腫瘤は閉鎖しており腹腔内との交通は認めなかった．子宮円索とともに高位結紮切離した後，Marcy法にて内鼠径輪の縫縮を加えた．摘出した腫瘤の内容物は暗赤色の液体で，大きさ6.3cm×2.6cmの薄い皮膜に覆われた単房性の腫瘤であった．病理組織学的検査で子宮内膜上皮成分が認められたことから，子宮内膜症を伴ったNuck管水腫と診断された．成人のNuck管水腫は比較的稀な疾患であるが，鼠径部腫瘤の鑑別として念頭に置く必要がある．
