Abstract
Background Scar endometriosis is a rare and often overlooked complication following cesarean section, presenting diagnostic challenges due to its variable clinical presentation. Classically, it manifests as cyclical pain and swelling near the surgical scar, but atypical cases without a palpable mass can easily be missed. Case presentation A 34-year-old woman reported pricking lower abdominal pain that worsened during menstruation. Multiple ul-trasounds were inconclusive until a small anechoic lesion was identified at the cesarean scar site, confirming scar endometriosis. Discussion This case highlights that scar endometriosis, though uncommon, can mimic other postoperative lesions and therefore requires careful clinical evaluation. From an Ayurvedic perspective, it corresponds to Raktaja Granthi, caused by Agantuja Abhighata and Rakta-Pitta Dushti, and therapies such as Jalauka Avacharana and Sheetala Upakrama are considered beneficial. Conclusion Atypical scar endometriosis demands strong clinical suspicion and imaging support. An Ayurvedic interpretation of Raktaja Granthi offers valuable complementary insights for diagnosis and care.
