This case report describes a 27-year-old woman with a prior history of ovarian endometriosis who presented with right shoulder/omalgia; thoracic CT identified a solid, ovoid, contrast-briefly enhancing lesion in the right subdiaphragmatic region, and MRI supported a diagnosis of ectopic endometriosis. She underwent right video-assisted thoracoscopic surgery (VATS) with diaphragmatic resection and primary repair, and pathology confirmed endometriosis foci with negative margins; a pleurostomy was removed after 48 hours and she was discharged after three days. At one year of follow-up, she was asymptomatic with no evidence of recurrence, and the authors highlight the low frequency and quality-of-life impact of thoracic endometriosis while noting that hormonal therapy is associated with recurrence and surgery is considered in refractory or complicated cases.
