Vitamin D levels in women with chronic pelvic pain syndrome in endometriosis

The study evaluated vitamin D (total 25-hydroxyvitamin D3) levels and their relationship to pelvic pain intensity in 210 women with externally genital endometriosis: 180 with chronic pelvic pain and 30 with endometriosis without pain, using a visual analog scale to quantify pain and dividing the pain group into weak, moderate, and severe subgroups. Mean serum vitamin D was lower in the pain group (22.23 ± 5.21 ng/mL) than controls (29.22 ± 4.23 ng/mL), with overall values averaging vitamin D insufficiency. Using Tukey’s criterion, vitamin D was significantly lower in the moderate pain subgroup versus weak pain (p = 0.005) and in severe pain versus moderate pain (p = 0.004), and correlation analysis showed an inverse association between pain intensity and serum vitamin D; the limitation is that it is cross-sectional and does not establish causality. Relevance to endometriosis: the paper specifically studied women with externally genital endometriosis and reports that lower vitamin D levels correlate with greater severity of chronic pelvic pain.
