This paper reports a 45-year-old woman with a pre-sacral mass lesion on MRI that extended into the left S1 exit foramen, compressing/displacing the S1 nerve and producing S1 radiculopathy, while not affecting S2 or S3. The lesion measured 6×2.5 cm and showed heterogeneous T1/T2 signal with focal T1 high signal consistent with hemorrhage, plus predominantly hyperintense T2 signal and relatively uniform enhancement; trans-sacral biopsy was performed to exclude recurrent rectal carcinoma. Biopsy confirmed endometriosis with endometrial glands and stroma and strong estrogen and progesterone receptor positivity, and neurological symptoms improved after hormonal treatment, with the authors noting that prolonged nerve compression can cause irreversible damage.
