This study analyzed IVF outcomes in 164 women with surgically diagnosed endometriosis and 112 controls with tubal factor infertility to determine how anatomical manifestations affect fertility mechanisms. Higher rASRM stages, particularly those involving deep infiltrating lesions or tubal pathology, were associated with reduced egg yield, lower anti-Müllerian hormone levels, and increased gonadotropin consumption, indicating diminished ovarian reserve. However, these factors did not significantly impair fertilization rates, embryo quality, or pregnancy success, while minimal peritoneal disease showed no significant fertility restrictions compared to controls.
