Abstract
Introduction Endometriosis is a benign condition affecting women's health. The pathogenesis is not well described but several theories have been proposed, including retrograde menstruation. Some conditions, such as uterine malformations, that may predispose to retrograde menstrual flow have been related to a higher risk of endometriosis. Main symptoms and/or clinical findings A 29-year-old woman with left congenital renal agenesis and a bicornuate uterus reported several years of dysmenorrhea and dyspareunia, and a left adnexal mass associated. Main diagnoses, therapeutic interventions and results Ultrasound imaging allowed a-left ovarian endometriosis and deep endometriosis diagnosis with a 3-year delay. Hormonal treatment with estro-progestogens combined oral contraceptives in an initial and extended flexible regimen was initiated for symptom relief and to prevent disease progression. Clinical but no radiological improvement was reached. Conclusion Uterine malformations, as a bicornuate uterus, are a predisposing risk factor for endometriosis, and for early onset and severe symptoms. Endometriosis is a condition with a high diagnosis delay, and 3-Dimensional ultrasonography is crucial for both uterine malformations and endometriosis characterization and diagnosis.
