This study enrolled 25 patients with clinical suspicion of ovarian endometriosis and compared ultrasonography (USG) and MRI findings, with demographic and clinical details recorded and both imaging modalities performed in all patients. On USG, thick-walled cyst with lower level echoes, hyperechoic wall foci, dependent fluid level, and negative sliding sign were reported in 92%, 60%, 48%, and 75% of patients, respectively, while MRI showed T1 hyperintensity with persistence on fat-suppressed T1, T2 hypointensity, T2 shading, STIR signal suppression, and DWI restriction with low ADC values in 96%, 96%, 80%, 64%, and 80% of patients, respectively. The authors found that MRI was superior to USG for locating endometriosis at more than two sites and for non-endometriosis diagnosis, while noting USG utility for initial screening, though the small sample size (25 patients) is a key limitation.
