This case report describes a 37-year-old woman with an incidentally discovered midline pelvic Müllerian duct cyst (MDC), evaluated using pelvic MRI that showed a well-circumscribed, non-enhancing posterior pelvic cystic lesion. Laparoscopic resection was performed, and intraoperatively dense adhesions were observed between the cyst, the posterior uterine wall, and the posterior peritoneum; histopathology confirmed MDC and identified coexisting endometriosis in the adherent posterior peritoneum. The main limitation is that, as a single-patient case report, the findings do not establish broader diagnostic or epidemiologic conclusions.
