Abstract
症例は37歳, 女性. 間歇的な下腹部痛が出現し当院受診. 右下腹部に強い圧痛があり筋性防御を伴っていた. 腹部超音波検査, CT検査にて終末回腸に壁肥厚があり, 口側腸管が拡張していた. 確定診断は得られなかったが, 症状が軽快せず貧血を伴い腫瘍性病変も否定できないことから手術を行った. 終末回腸に硬く触知する腫瘤が存在し狭窄を呈していた. 子宮付属器に明らかな異常はなかった. 炎症性病変による回腸狭窄と判断し回盲部切除術を行った. 切除標本では終末回腸粘膜に限局性の浮腫があり, 狭窄を呈していた. 漿膜筋層は硬く瘢痕様であった. 病理組織診にて, 回腸漿膜下層に間質を伴う子宮内膜腺管が存在し, 回腸子宮内膜症の診断であった.
