This case report describes abdominal wall invasion involving musculature by a wall endometrioma in a 38-year-old patient with chronic pelvic pain and irregular vaginal bleeding, assessed using exploratory laparotomy and total hysterectomy with removal of a mass suspected to be malignant based on imaging. Intraoperative concern for adjacent structure involvement led to a multidisciplinary approach, while definitive diagnosis was established by histopathology confirming endometrioma and excluding the malignancy suggested on imaging. Biopsy of the excised mass also ruled out sarcoma of the uterus and desmoid tumor, and the authors highlight the condition’s rarity and diagnostic and treatment challenges as key considerations.
