Prognostic value of two morphological forms of chronic endometritis: polypoid and follicular one

This study compared two clinico-morphological forms of chronic endometritis—polypoid (n=214) versus follicular (n=31)—in 245 women with failed IVF attempts, using histological and immunohistochemical analysis of endometrial samples collected in the early follicular phase (days 5–9) and standardized combined therapy. The follicular form showed significantly higher counts of NK cells (CD56, CD16), activated immune cells marked by HLA-DR II, B lymphocytes (CD20), and plasma cells (CD138) than the polypoid form. After treatment, pregnancy occurred in 12/31 with follicular endometritis and ongoing pregnancies leading to live birth occurred in 5/31, with early pregnancy loss being nearly sixfold higher in the follicular group (OR 5.8; 95% CI 1.8–15.1), while overall pregnancy absence and ongoing pregnancy rates did not significantly differ. The paper notes the need for further study to clarify mechanisms and develop targeted treatment.
