This case report evaluated the diagnostic utility of fine needle aspiration cytology (FNAC) with cell block preparation for a suspected scar endometriosis mass in a 27-year-old woman who developed a progressively enlarging, ~5×5 cm nodule at a prior caesarean section scar 4 years after surgery, with cyclical pain aligned to menses. FNAC smears showed hemorrhage, scattered small cells resembling endometrial stromal and columnar epithelial elements, and hemosiderin-laden macrophages, leading to a cytology diagnosis suggestive of scar endometriosis, while confirmation was obtained through cell block histology and final excisional histopathology. The main limitation is that this evidence is drawn from a single patient case rather than a comparative or controlled study.
