Abstract

症例は33歳，女性．19年前に腹膜炎合併虫垂炎にて虫垂切除術を施行されていた．2006年12月頃より虫垂切除術後創部の疼痛が時々出現，2010年9月頃より創部の疼痛が増悪し，同部の皮下結節を触知するようになった．結節は径約1.5cmで可動性はなく弾性硬であった．術前検査にて筋膜縫合部の縫合糸を原因とするSchloffer腫瘤が疑われ，腰椎麻酔下に切除術を施行した．病理組織学的所見で切除組織内に子宮内膜組織を認め，子宮内膜症と診断された．子宮内膜症のうち，手術後の腹壁に生じる腹壁瘢痕部子宮内膜症は，約80％が帝王切開などの婦人科領域の手術既往によるものであるが，本症例は虫垂切除後の腹壁瘢痕部に発生したものであり，稀有な症例であるため，報告した．
