Uterotubare Transportstörung bei Adenomyose und Endometriose – Ursache der Infertilität
Endometriosis and adenomyosis are considered different manifestations of a shared underlying condition: misplaced basal endometrium that disrupts uterotubal transport, leading to infertility.

This study investigated uterotubal transport dysfunction as a cause of infertility in women with endometriosis and adenomyosis, utilizing MRI to diagnose adenomyosis via junctional zone widening and hysterosalpingoscintigraphy to assess sperm transport capacity. The results indicated that patients with diffuse adenomyosis exhibited absent sperm transport in 93% of cases, while those without radiological evidence of adenomyosis maintained intact transport function in 80%. Furthermore, negative transport findings excluded the possibility of spontaneous or intrauterine pregnancy by 90%, highlighting the uterus's central role in the pathogenesis where dysperistalsis destroys the subendometrial layer necessary for regulated sperm movement. This paper is centrally about endometriosis and adenomyosis, specifically examining how adenomyosis associated with endometriosis disrupts uterotubal transport to cause infertility.
