Functioning Endometrium and Endometrioma in a Patient with Mayer-Rokitanski-Kuster-Hauser Syndrome

This case report describes a 27-year-old woman diagnosed with Mayer-Rokitanski-Kuster-Hauser (MRKH) syndrome who had primary amenorrhea and cyclic lower abdominal pain, with MRI showing Müllerian remnants and a right 7.0 cm endometriotic cyst. The authors performed laparoscopic resection of the Müllerian remnants and right ovarian endometrioma, plus vaginoplasty, and intraoperative/pathologic findings included functional small endometrium within non-canalized uterine horns and an endometriotic cyst. Continuous combined oral contraceptives did not relieve symptoms preoperatively, and the authors note that the pathogenesis of endometriosis in MRKH may be multifactorial rather than explained by a single theory.
