This paper reports a case of primary ovarian adenomyoma, an extremely rare extra-uterine adenomyoma, in a 45-year-old woman who also had an endometrial polyp. Using transvaginal ultrasonography, the authors describe a solitary endometrial polyp and an enlarged left ovary with heterogenous echogenicity, followed by surgical management with total abdominal hysterectomy and bilateral salpingo-oophorectomy. Histologic examination confirmed an adenomyoma arising primarily in the ovary, and the authors frame the report as the fourth documented case in the literature while acknowledging that only a handful of cases exist, limiting generalizability. This paper is centrally about endometriosis and/or adenomyosis-related pathology — it focuses on primary ovarian adenomyoma arising in the ovary in a patient with an endometrial polyp, placing it within the broader adenomyosis spectrum.

Abstract

Background An adenomyoma presenting outside the uterus is an extremely rare entity and only three cases of primary ovarian adenomyoma have been reported up to date. Case report We report the fourth case of ovarian adenomyoma in a 45-year-old woman with an endometrial polyp. Transvaginal ultrasonography revealed a solitary endometrial polyp with an enlarged left ovary which appeared heterogenous with isoechoic and mildly hyperechoic pattern.

Conclusion

Total abdominal hysterectomy and bilateral salpingo-oopherectomy was performed and histologic examination revealed an adenomyoma arising primarily in the ovary.
