Endometriosis with colonic and rectal involvement: surgical approach and outcomes in 142 patients

This retrospective study reviewed consecutive surgically managed cases of endometriosis involving the colon and/or rectum in 142 patients treated between 2010 and 2018, evaluating associations between surgical approach, 30-day complications, symptom improvement, and time to recurrence using multivariable logistic regression. Patients underwent mostly superficial partial-thickness excision (66.9%), with higher complication rates linked to greater blood loss, sigmoid involvement, stoma formation, and segmental resection, while partial-thickness or disc-type excision (partial-thickness/full-thickness) was associated with fewer minor complications but differed from bowel resection in recurrence patterns. Recurrence occurred in 19.1%, with open surgery and superficial partial-thickness excision associated with recurrence, whereas segmental resection was associated with decreased recurrence risk; the authors’ key caveat is the retrospective design.
