症例は35歳, 女性. 子宮内膜症で婦人科に通院中であり, 月経前に便秘が出現していた. 腹部膨満が出現したため入院となり, 大腸イレウスと診断された. 保存的治療を開始したが改善せず, 手術の方針となった. 直腸に手拳大の腫瘤性病変を認め, ハルトマン手術とした. 切除標本の病理組織学的検査で直腸子宮内膜症と確定診断された.大腸の子宮内膜症による腸閉塞の本邦報告は過去に38例のみであったが, 子宮内膜症の既往のある性成熟期の女性で, この疾患の可能性を念頭に置き, 月経に随伴する症状の詳細な聴取を行うことが必要である.
