Abstract

症例は45歳，女性．2012年12月，月経終了後より便秘傾向となり，嘔吐・腹痛のため当院内科外来を受診した．腹部CT検査よりS状結腸に高度狭窄を認めたため，経肛門的にイレウス管の挿入を試みるも，施行困難と判断し，外科紹介となった．同日，緊急手術を施行した．回腸および全結腸が著明に拡張しており，S状結腸に腫瘤を触れ，直腸および子宮に強度に癒着していた．S状結腸切除，D2郭清を施行した．術後，病理組織学的診断により腸管子宮内膜症と診断された．腸管子宮内膜症は，全子宮内膜症の10％程度を占め，S状結腸・直腸での発生頻度が高い．しかし，同部位でのイレウスの発症は比較的稀と報告されている．今回，S状結腸に発症しイレウスをきたした腸管子宮内膜症の１例を経験したので報告する．
