Background: Endometriosis is a chronic gynecological condition that significantly affects women’s quality of life and poses diagnostic challenges in primary care settings. Aim and Objectives: This study aims to assess the knowledge, attitudes, confidence, and perceived barriers regarding endometriosis among primary healthcare physicians in Riyadh, Saudi Arabia. Methods: A cross-sectional analytical study was conducted using a structured questionnaire administered to 215 physicians working in hospitals and primary healthcare centers in Riyadh. The questionnaire covered four domains: knowledge, attitudes, confidence, and barriers. The reliability of the instrument was assessed using Cronbach’s alpha coefficient, which yielded a value of 0.86, indicating good internal consistency. Results: The findings showed that primary healthcare physicians had a relatively high level of knowledge, positive attitudes, and a high degree of professional confidence in managing endometriosis. Significant positive correlations were found between knowledge, attitudes, confidence, and quality of care. In contrast, perceived barriers were negatively associated with quality of care. Multiple linear regression analysis revealed that professional confidence was the strongest predictor of quality of care, followed by attitudes and knowledge, while barriers had a significant negative impact. The model explained a significant proportion of variance in care quality.
