El tumor con patrón villoglandular es un subtipo poco frecuente del adenocarcinoma endometrioide, considerado de esta manera desde el año 1998 por su frecuente presentación mixta con el carcinoma endometrioide típico y sus características biológicas similares. Es comúnmente identificado en mujeres postmenopáusicas cuyo síntoma principal es la metrorragia y se presenta de manera precoz permitiendo identificarlo de manera oportuna para su tratamiento y pronóstico. Existen características histológicas (mioinvasión) que pueden estar relacionadas con un pronóstico diferente; sin embargo, no han sido reconocidas de manera oficial. Se presenta el caso de una paciente con resultado histopatológico de esta variante, pero muy mal pronóstico debido al estadío clínico identificado, a pesar del buen grado de diferenciación, dado su negativa para recibir tratamiento.
