Abstract

症例は43歳,女性.腹痛,発熱,水様便につづく下血を主訴に来院した.下血は最終月経初日から6日目に起きた.下部消化管内視鏡検査および注腸造影で虫垂開口部に壁外性の圧迫像をみとめ,腹部CTおよびMRIで同部に21×18mm大の造影効果のある辺縁平滑な腫瘤を描出した.虫垂腫瘍の診断で回盲部切除術を施行したところ,虫垂根部に白色の充実性腫瘍をみとめ,内部に出血斑と思われるヘモジデリン沈着を伴った.病理組織検査の結果,固有筋層から漿膜下にかけて子宮内膜間質組織と内膜腺の増生をみとめ,虫垂子宮内膜症と診断した.
