症例は48歳, 女性。左鼠径部膨隆に対し鼠径ヘルニアと診断され入院。術前採血では小球性貧血以外に異常を認めず。鼠径部超音波では鼠径管に一致してlowe choic mass が認められた。左鼠径ヘルニアを最も疑い手術を施行しところ, 内鼠径輪にヘルニア嚢が認められる以外に, 鼠径管内を占める形で凝血塊様の腫瘤が認められ円靱帯との境界は不明であった。ヘルニア嚢は内鼠径輪の高さで周辺より剥離し高位結紮し, 腫瘤は円靱帯とともに切離した。PHS mesh にてrepairし手術終了…
