Abstract

Abstract Background Diabetes is a leading cause of death and disability in lower-middle-income countries, including Bangladesh, where the rising prevalence of type-2 diabetes poses a significant burden on healthcare systems and individuals. This study aimed to evaluate the knowledge about type-2 diabetes among university students in Bangladesh. Material and Methods A cross-sectional survey was conducted at the Bangladesh University of Health Sciences from October to December 2023. A total of 380 undergraduate students from three faculties participated in this study. Data were analyzed using one-way ANOVA to explore mean difference and multivariate logistic regression model to identify significant predictors of knowledge. Results Overall, 60.5% of students had above-average knowledge about type 2 diabetes. Factors such as gender, residence, academic performance, and family history of diabetes significantly influenced overall knowledge (P < 0.05). Domain-specific knowledge was also shaped by gender, residence, source of information, and faculty. Better academic performance (AOR:1.96, CI:1.22–3.14) and family history of diabetes (AOR:1.97, CI:1.21–3.21) were more likely to have higher knowledge than their counterparts. Conclusion A large portion of students had below average knowledge about type-2 diabetes. Targeted health education and awareness programs are crucial to address knowledge gaps, particularly among rural students and those without family exposure to diabetes.

Type-2 Diabetes Knowledge University Students Bangladesh Figures Figure 1 1 Introduction Diabetes is a global public health concern that contributes to rising death and disability [ 1 , 2 ]. As of 2021, the prevalence of diabetes among adults aged 20 to 79 was 10.5%, primarily type 2 diabetes, with projections suggesting it may reach to 12.2% by 2045. About 81% of those were in middle- and low-income countries [ 1 , 2 ]. The type-2 diabetes, while most prevalent in older adults, is increasingly affecting adolescents and young adults, that can be largely prevented through lifestyle modifications [ 3 – 9 ]. A recent survey in Bangladesh revealed a growing prevalence of diabetes among young adults aged 18 to 34. The rates increased from 5–9% and 5–8% was among young women and men respectively [ 10 , 11 ]. Early onset of diabetes leads to longer exposure to hyperglycemia, increasing the risk of long-term complications and reducing quality of life [ 3 , 5 , 12 ]. Diabetes significantly affects global health expenditures, representing 11.5% of total spending [ 1 ]. In Bangladesh, the healthcare system relies heavily on out-of-pocket costs, which rose from 67–73% within 2015–2021 [ 13 ]. Individuals with diabetes face healthcare costs that are six times higher than those without the condition, spending an average of 9% of their annual household income [ 14 , 15 ]. Diabetes is a chronic illness that demands long-term treatment and can incur high costs. In the absence of universal health insurance, prioritizing prevention and cost-effective management is essential [ 14 , 16 ]. A study in Bangladesh depicted that diabetic people receive fewer preventive services and medications, leading to higher inpatient care rates [ 14 ]. Therefore, effective diabetes prevention and management are crucial for reducing healthcare costs and improving quality of life. To meet the Sustainable Development Goals by 2030, we must meet the targets of reducing premature mortality from non-communicable diseases, including diabetes, by one-third [ 17 ]. Health-related knowledge and awareness are crucial for adopting positive habits in youth, which can lead to lifelong practices [ 18 , 19 ]. Healthy dietary choices and exercise are key for managing and preventing type 2 diabetes, as they are linked to a lower risk of the disease [ 20 – 22 ]. In Bangladesh, approximately 46 million youth aged 15–29 make up nearly one-fourth of the population with university students playing a significant role in public health advocacy [ 23 ]. Therefore, assessing their knowledge about type 2 diabetes—including its risk factors, complications, signs, and symptoms—is essential for empowering them to protect themselves and their families and communities. Research in various developing countries has examined knowledge, attitudes, and perceptions of type 2 diabetes among students. While some studies indicated satisfactory basic knowledge, many students lacked understanding of signs, symptoms, and control measures [ 24 – 28 ]. Other studies reported moderate to good knowledge, positive attitudes, and high awareness [ 27 , 29 – 32 ]). Factors significantly influencing diabetes knowledge included gender [ 26 , 28 , 30 , 33 , 34 ], residence [ 24 , 27 , 34 – 36 ], field of study [ 27 , 28 , 34 , 37 , 38 ], academic performance [ 7 , 39 ], prior diabetes information [ 38 , 40 ], family history [ 7 , 26 , 28 , 30 ], and lifestyle choices like physical activity [ 28 , 41 ] and healthy diet [ 28 ]. Recent studies in Bangladesh have examined the knowledge and awareness about type-2 diabetes among diabetic patients and the general population [ 36 , 42 – 44 ]. However, there has yet to be any research focused on university students. This study aims to evaluate both overall and specific knowledge about type-2 diabetes among university students and to identify associated factors. Our findings may help develop an effective strategy to enhance knowledge of type-2 diabetes among university students. 2 Materials and Methods The cross-sectional study was conducted at the Bangladesh University of Health Sciences from October to December 2023. Located in Mirpur-1, Dhaka, this is the country's first health sciences institution, comprising four faculties: Basic Sciences, Public Health, Health Engineering, and Allied Health Sciences. Basic Sciences, Public Health, and Health Engineering together offer twelve undergraduate and graduate programs, while Allied Health Sciences provides one graduate program. Data were collected from second to fourth-year undergraduate students, excluding foreign students. The target sample size was 403, based on a knowledge prevalence of 0.48 with a 5% non-response rate [ 37 ]. A semi-structured questionnaire was developed after reviewing relevant literature and consultation with epidemiologists and public health specialists, and it was tailored to the local context. It was pretested with eight participants to identify comprehension issues, and revisions were made based on feedback. The reliability of the questionnaire was assessed using Cronbach's alpha, yielding a value of 0.85. The questionnaire included sections on socio-demographic information, lifestyle factors, and knowledge questions. A total of 560 students were invited. Finally, 380 (68%) respondents given consent and interviewed accordingly. Knowledge about type 2 diabetes was the dependent variable. The knowledge was assessed using a 32-item questionnaire covering general knowledge, diabetes related risk factors, symptoms, complications, diagnosis, and treatment. A score of '1' was given for each correct answer and '0' for incorrect answer. The study considered various socio-demographic and lifestyle factors as independent variables. These included participants' gender, residence (rural, urban, or semi-urban), living situation (with family or not), educational background, faculty (Basic Sciences, Public Health, or Health Engineering), academic performance (classified as "poor" if CGPA is below 3.50, and "good" if it is 3.50 or higher), sources of information (academic lectures, seminars, media, or others), family history of diabetes, and lifestyle factors such as regular breakfast, sleep duration, and physical activity. All statistical analyses were performed using Stata version 18. All outcome variables—including general knowledge, diabetes related risk factors, symptoms, complications, diagnosis, and treatment—were summarized using means and standard deviations (Mean ± SD). The mean differences between groups were assessed using analysis of variance (ANOVA). A multivariate binary logistic regression model identified factors influencing knowledge about diabetes. Respondents were considered knowledgeable if they answered “Yes” to more than three of six items in each domain; those who answered fewer than four were categorized as lack of knowledge. For the overall knowledge score, respondents were classified as knowledgeable if their score was above average and as lacking knowledge if below. The P-value P < 0.05 was considered as significant. Ethical declaration and consent statement : This study approved by the ethical review committee of Bangladesh University of Health Sciences with Memo No: BUHS/ERC/EA/23/57, dated on 25th October 2023. We followed each steps of Helsinki declaration regarding ethical approval and consent to participate. After sharing of purpose, risks and benefits, freedom to participate or reject, withdrawal at any time, estimated time of interview, privacy and anonymization, use of data and other relevant information according to Helsinki declaration, a written informed consent was obtained from each participant. 3 Results Out of 380 students, overall above average knowledge about type 2 diabetes rate was 60.5% than their counterpart (Fig. 1 ). Around 53%, 29%, 49% was male, from rural and urban areas respectively. A portion of respondents (22%) were from semi-urban areas. Interestingly, semi-urban residents had higher knowledge than their counterparts (P < 0.05). Among the surveyed students, 43% were from the basic science faculty, 21% from public health, and the rest of respondents were from health engineering faculty. Public health students had highest knowledge than their counterparts (P < 0.01). A positive correlation exists between academic performance and diabetes knowledge; students with CGPA 3.50 or higher demonstrated greater knowledge than those with lower CGPA (P < 0.01). The respondent’s family history of diabetes had significant impact on improving knowledge. Improved lifestyle, such as regular breakfast and physical activity, influence diabetes knowledge as well. About 50% of respondents regularly skipped breakfast, while around 41% exercised at least five days a week, with the more active individuals typically showing higher levels of knowledge about diabetes (Table 1 ). The multivariate binary logistic regression analysis examines the influence of socio-demographic and lifestyle factors on knowledge about type-2 diabetes among university students. According to education background, the students with both a Higher Secondary Certificate (HSC) and a diploma had two times higher diabetes knowledge than those with only an HSC. Students with a cumulative GPA 3.50 or higher were more likely to be knowledgeable about diabetes than their counterparts (AOR:1.96, CI:3.14–1.22), and those with a family history of the disease were more likely to understand it (AOR:1.97, CI:3.21–1.21), both significant at the 1% level. In contrast, factors such as being from urban areas, studying in public health faculty, and maintaining healthy habits—like regularly eating breakfast and exercise for at least five days a week—showed higher levels of diabetes related knowledge, but these relationships were not statistically significant (P > 0.10). This suggests that while some socio-demographic and lifestyle factors influence health awareness, they do not lead to meaningful differences in diabetes knowledge (Table 2 .). SD = Standard, CI = Confidence Interval. The multivariate binary logistic regression highlighted the influence of socio-demographic and lifestyle factors on domains specific knowledge among students. Female students showed higher knowledge of diabetes symptoms (54%) and had significantly better understanding of its treatments. The urban students were generally possessing more knowledge than rural students, particularly regarding the risk factors and treatment of diabetes. Subsequently, the public health faculty and academic performance such as the respondents with CGPA of 3.50 or above exhibited better knowledge in major areas. Students with a family history of diabetes were 2.46 times more likely to understand the risk factors than their counterparts (P < 0.01) (Table 3 ). The academic lecture, diabetes related seminars, media advertisement, posters and other sources such as peers, relatives, contact with patients, newspaper and articles developed their knowledge about risk factors, symptoms, complication, diagnostic sources and treatment of diabetes. Students who obtained both Higher Secondary Certificate (HSC) with a diploma prior to admission at university exhibited a greater level of current knowledge. It is noted that Bangladesh University of Health Sciences (BUHS) is a multidisciplinary university accepting students who have at least HSC degree with or without diplomas. Our finding indicates that the background education with supplementary educational diploma enhanced both academic and individual health related knowledge. Similar conclusions were drawn from a study conducted in Ethiopia [ 38 ]. Students with strong academic performance were more likely to have better knowledge about diabetes, suggesting that a studious student is brilliant everywhere. Besides the academic study, they might have diverse learning enthusiasm. Another study in China validated our findings [ 7 ]. The students with a family history of diabetes were more likely to be knowledgeable about the disease indicates the disease itself educates family members. As diabetes shaped the life-style of the family members (father, mother, elder brothers or sisters, grand-father or grand-mother) and support them to maintain healthy behavior such as avoidance of fatty food, junk food, excess food, excess carbohydrate, start walking, physical exercise, avoidance of stress and smoking, regular health checkup. By observing such changing life-style improved the knowledge and understanding other family members such as university students. Our findings were aligned with previous studies [ 7 , 28 , 30 ]. The female students exhibited comparatively a higher level of knowledge about diabetes than male students which is consistent with other studies [ 26 , 28 ]. It reinforces the notion that women generally possess better health literacy, likely due to their greater involvement in health-related discussions, exposure to information and proactive health-seeking behaviors. Furthermore, in Bangladesh context, female members particularly young members are usually involved with caring the family members according to guidance of physicians. Hence, they try to learn necessary advice of the physician, coordinate and timely inform the diseased person that he/she need to walk, or take medicine or serving required quantity of food to him/her. Hence, they gather knowledge through learning by doing. The students coming from urban areas had greater understanding of treatment options and risk factors than their counterparts. This might be attributed to the easier access to healthcare information and services available in urban environments, a trend that has been depicted in previous research [ 22 ]. The urban students have comparatively diverse sources of acquiring knowledge such as observing morning exercise, a frequent discussion issue of parents, friends or relatives, frequent mass media advertisement, health sessions within or outside the academic environment that impacts on improving knowledge about diabetes. Furthermore, students who participated in seminars or symposiums were more likely to possess knowledge related to diabetes. It means, to be healthy and to maintain healthy life-style, seminars or symposiums impacts more over formal academic learning. This study also suggests that attending these events can enhance students' comprehension of diabetes-related topics. Other studies strengthen our findings [ 36 , 38 ]. The students who engaged in regular physical activity contributes to an increased awareness of diabetes, indicating that a health-conscious lifestyle can fit them physically and mentally healthy and can reduce the risks of non-communicable diseases including diabetes [ 28 ]. Limitations and strengths of the study Comparatively low sample size and minimum variables were set in this study. Parent’s occupation, income, smoking, family size, religion and other life-style and socio-economic and socio-demographic factors were not properly explored. Furthermore, the respondents were selected using non-probability sampling that cannot generalize the overall results for all students. In light of the limited existing literature, we have also drawn upon additional studies conducted among non-diabetic adults to enrich our findings. While our approach was a convenience sampling method, we made a concerted effort to include participants from all faculties and departments. To the best of our knowledge, this is the first study at this university to assess knowledge of type 2 diabetes. 5 Conclusion Around 40% students had below average knowledge about diabetes. This study underscores that the education background, academic performance, and family history of diabetes improve the knowledge about Type-2 diabetes among university students. Subsequently, gender, residence, and sources of information affected the knowledge difference. These findings highlight the critical gaps in diabetes awareness among certain student groups, emphasizing the need for tailoring the interventions to address these disparities. To achieve health targets of sustainable development goals (SDG) for non-communicable diseases (NCD) including diabetes, this study recommends regularly organizing seminars, workshops, and symposiums to minimize knowledge gaps among university students. Furthermore, integrated health-related topics into broader university curricula and access to credible information sources would promote self-directed learning. This study aimed to evaluate the knowledge about type-2 diabetes among university students in Bangladesh.\u003c/p\u003e\u003ch2\u003eMaterial and Methods\u003c/h2\u003e\u003cp\u003eA cross-sectional survey was conducted at the Bangladesh University of Health Sciences from October to December 2023. A total of 380 undergraduate students from three faculties participated in this study. Data were analyzed using one-way ANOVA to explore mean difference and multivariate logistic regression model to identify significant predictors of knowledge.\u003c/p\u003e\u003ch2\u003eResults\u003c/h2\u003e\u003cp\u003eOverall, 60.5% of students had above-average knowledge about type 2 diabetes. Factors such as gender, residence, academic performance, and family history of diabetes significantly influenced overall knowledge (P\u0026thinsp;\u0026lt;\u0026thinsp;0.05). Domain-specific knowledge was also shaped by gender, residence, source of information, and faculty. Better academic performance (AOR:1.96, CI:1.22\u0026ndash;3.14) and family history of diabetes (AOR:1.97, CI:1.21\u0026ndash;3.21) were more likely to have higher knowledge than their counterparts.\u003c/p\u003e\u003ch2\u003eConclusion\u003c/h2\u003e\u003cp\u003eA large portion of students had below average knowledge about type-2 diabetes.
