Objective : To evaluate the risk factors for assisted vaginal births (AVB) and to compare perinatal factors and outcomes of vacuum extraction applied at the mid/low cavity and at the perineum. Study design : Retrospective observational monocentric study conducted on the cohort of women who delivered vaginally in 2023 at the Fondazione IRCCS Ca' Granda Ospedale Maggiore Policlinico in Milan. Results : A total of 3833 vaginal births were included: 3090 uneventful vaginal births and 365 assisted vaginal births. After adjusting for all factors considered, maternal age (OR 1.04, 95% CI: 1.01-1.07), mode of conception (OR 1.58, 95% CI: 1.07-2.33), epidural analgesia (OR 6.25, 95% CI: 3.05-12.80), gestational age (OR 1.48, 95% CI: 1.31-1.67), and newborn male sex (OR 1.35, 95% CI: 1.06-1.73) were positively associated with AVB, whereas parity (OR 0.20, 95% CI: 0.14-0.29) was inversely associated.Compared to perineal applications, mid/low pelvic applications were associated with greater blood loss (p<0.01), higher episiotomy rate (95.5% vs 88.2%, p=0.03), and increased ultrasound use (62.4% vs 23.5%, p<0.01). Conclusions : Increased maternal age, nulliparity, medically assisted conception, epidural analgesia, gestational age, and newborn male sex were identified as potential risk factors for AVB. Vacuum extractions performed at mid/low pelvic stations were associated with higher use of episiotomy and greater blood loss. These findings may help in risk awareness and a personalized labor management. Background Assisted vaginal birth (AVB) refers to the obstetric procedure commonly performed during the second stage of labor, aimed to expedite delivery after a proper evaluation of the alternative options of cesarean birth or expectant management. A wide range in the AVB prevalence has been described worldwide, ranging from 3 to 15% of all births, suggesting that inadequate or randomly applied evidence-based guidelines, as well as various familiarity and expertise with the technique are detectable in some areas 1 . In particular, the AVB rate has decreased over the past few decades, as a result of increased second stage cesarean section rates, despite its well known association with higher maternal and neonatal morbidity, 2 including impacted fetal head, neonatal trauma, skull fracture, intracranial hemorrhage, neonatal acidemia 3 , and neonatal intensive care unit (NICU) admission, as well as postpartum hemorrhage, surgical injury and third- or fourth-degree perineal tears 4 . In Italy, AVB is usually performed through the vacuum extractor as the first-choice technique compared to the forceps. 1 , 5 In order to ensure a tailored approach to AVB, it is crucial to evaluate ante- and intrapartum factors potentially increasing the risk of second stage dystocia, AVB and AVB failure, including both maternal and fetal characteristics, and factors associated with labor progression. Equally, fetal head position and station carefully evaluated as a prerequisite for successful AVB. Consequently, the use of intrapartum ultrasound could be of paramount importance for AVB success, particularly in case of mid cavity applications 6 , 7 , 8 . The aim of the present study was to retrospectively investigate antepartum and intrapartum risk factors for AVB. Secondly, we aimed at evaluating associated factors and outcomes according to the vacuum application at the perineal level versus mid/low pelvic stations. Materials and methods Study design, Setting and Population This was a retrospective observational monocentric study carried out between January 1st, 2023 and December 31st, 2023 at the Department of Obstetrics, Fondazione IRCCS Ca' Granda Ospedale Maggiore Policlinico, Milan, as a tertiary Maternity unit counting approximately 6000 births per year. All pregnant women who delivered vaginally in the study period were included in the analysis with no exclusion criteria. Clinical data were extracted from medical registry. Groups were defined based on mode of vaginal delivery (uneventful vaginal delivery versus AVB) and the latter was further divided into subgroups of perineal versus mid-low cavity AVB. For each included patient, the following data were recorded: maternal age, ethnicity, pregestational body mass index (BMI), parity, conception mode, pregnancy complications, gestational age at delivery, epidural analgesia use, oxytocin use, labor onset (i.e., spontaneous or induced), delivery mode (i.e., uneventful or AVB), episiotomy, perineal tears, estimated blood loss, neonatal birthweight, Apgar score at 5 minutes, arterial and venous BE and pH, and neonatal complications (cephalohematoma, intracranial hemorrhage, neonatal resuscitation maneuvers, NICU admission). In case of AVB, fetal head level and position, duration of second stage of labor, indication to the vacuum application as well as use of intrapartum ultrasound were reported. Statistical analysis Selected characteristics of the study population, including maternal, perinatal and neonatal factors, were summarized using descriptive statistics. Categorical data were presented as absolute and relative frequencies (percentages), and continuous data as medians and ranges. Maternal and neonatal characteristics according to the vaginal delivery mode (i.e., uneventful or AVB) were compared using Fisher’s exact test or Chi-square test for categorical variables as appropriate, whereas continuous variables were compared using Mann Whitney-U test. The antepartum and intrapartum factors examined included maternal age, pregestational BMI, parity, mode of conception, labor induction, epidural analgesia, gestational age at delivery, birth weight, and neonatal sex. Simple logistic regression models were used to estimate the unadjusted odds ratio (OR) and corresponding 95% confidence interval (CI) for AVB according to each factor, while multiple logistic regression was used to obtain the adjusted OR accounting for all factors considered. A secondary analysis was performed, including only AVB, to compare subgroups based on perineal plane and low-/mid-cavity vacuum application. This analysis aimed to investigate differences in maternal and fetal characteristics, perinatal factors, such as duration of second stage, use of intrapartum ultrasound, use of oxytocin and episiotomy and frequency of adverse outcomes, such as perineal tears, blood loss, neonatal acidosis (venous pH ≤ 7.1 and/or BE > -12), NICU admission, neonatal resuscitation maneuvers, cephalohematoma, and intracranial hemorrhage. Statistical tests employed included Fisher’s exact test, Chi-square test, and the Mann-Whitney U test. The study was conducted in agreement with the ethical standards for research involving human subjects established by the Declaration of Helsinki. Given the retrospective nature of the study and the patient anonymity, informed consent was not required. Results In 2023, 5921 deliveries took place in our Delivery Ward. Of these, 3833 were vaginal deliveries and were included in the present analysis, further divided into 3090 uneventful vaginal deliveries and 365 AVB. Among the AVB cases, 201 (55.2%) were performed due to fetal distress, while 164 (44.8%) as a result of failure to progress. There were no cases of vacuum extraction failure resulting in cesarean section. Table 1 provides the distribution of selected maternal, neonatal and perinatal characteristics according to vaginal delivery mode (i.e., uneventful or AVB). Women of the AVB group were more frequently nulliparous than those of the uneventful vaginal delivery group and more likely to have undergone assisted reproductive technologies to conceive. Epidural analgesia was administered in almost all of the women who had an AVB. As regards neonatal characteristics, babies from AVB were born at a greater gestational age and were more likely to be male. There were no differences in maternal age, pregestational BMI, induction of labor, and birth weight between the two groups. Table 2 shows OR and 95%CI for AVB according to selected maternal, neonatal, and intrapartum characteristics. Parity was associated with a reduced risk of AVB. Induction of labor, epidural analgesia, gestational age, medically assisted conception and newborn male sex were associated with increased risk of AVB. When accounting for all included predictors, maternal age (OR 1.04, 95% CI: 1.01–1.07), parity (OR 0.20, 95% CI: 0.14–0.29), mode of conception (OR 1.58, 95% CI: 1.07–2.33), epidural analgesia (OR 6.25, 95% CI: 3.05–12.80), gestational age (OR 1.48, 95% CI: 1.31–1.67), and newborn male sex (OR 1.35, 95% CI: 1.06–1.73) remained associated with AVB. No differences were detected in indication for vacuum application and maternal characteristics (i.e., age, BMI, weight gain, parity, chronic or pregnancy-related disease). Operative deliveries with vacuum extractor placed in the mid/low cavity were more frequently monitored with the use of intrapartum ultrasound (62.4% versus 23.5%, p < 0.01). With regard to perinatal factors and outcomes, episiotomy was performed more frequently in women who had a vacuum application at the mid/low cavity than in those at the perineal plane (95.5% versus 88.2%, p = 0.03), and mean blood loss was higher in the mid/low than in the perineal group (p < 0.01). Discussion The present study showed that maternal age, medically assisted conception, epidural analgesia, gestational age, and newborn male sex were positively associated with AVB, whereas parity was inversely associated and that applications at the mid/low pelvis were associated with greater blood loss, a higher use of episiotomy and increased use of ultrasound diagnosis. In our study, epidural analgesia emerged as the factor most strongly associated with AVB. Several studies, notably a 2018 Cochrane review 9 , demonstrated a potential association between epidural use and AVB risk. It has been hypothesized that this may be related to the role of epidural analgesia in relaxing the pelvic floor muscles and reducing maternal pushing perception, ultimately contributing to the need of AVB. 9 – 13 However, the same review highlighted that the entity of this association remains controversial due to the poor data quality of the included studies. Regarding maternal age, the univariate analysis did not reveal significant differences between women who underwent AVB and those who had a spontaneous vaginal delivery. However, the multivariate analysis showed a 4% increase in the likelihood of AVB for each additional year of age, confirming a well-established finding. 5 , 9 – 12 , 14 It is challenging to pinpoint the exact causes of this association. Two primary mechanisms could be proposed: on one hand, intrinsic biological factors, including more efficient uterine contractions and greater physical resilience in young women 12 ; on the other hand, a likely greater tendency towards medicalization of labor in older women. 14 Our data also showed an approximately 80% reduction in the risk of AVB in multiparous women. This association may be attributed to biomechanical and anatomical factors specific to nulliparous women, such as a less distensible perineum, a birth canal that has not yet adapted to labor processes, and typically slower fetal descent compared to multiparous women. These factors can contribute to prolonging the second stage of labor, potentially leading to AVB. 5 , 15 Interestingly, male fetal sex increased the risk of AVB by 50%. This finding emerged in both univariate and multivariate analyses and has been recently reported by other research groups as well. 5 , 16 , 17 Among the speculations regarding the potential explanations, some authors suggested that the higher incidence of macrosomia in male fetuses may be a contributing factor. However, in our study the association remains significant even after adjusting for birth weight in multivariate analysis, indicating that the association between male fetal sex and the risk of AVB is independent of birth weight. Other authors reported that another factor potentially contributing to a longer duration of labor and a higher incidence of AVB in male fetuses could be the presence of greater head circumference or the different adaptation that male fetuses exhibit in case of adverse conditions such as during labor 16 , 17 , 18 . However, further evidence is needed to confirm those hypotheses. In the secondary analysis, we divided the population into two subgroups based on the digital station of the fetal head at the time of vacuum application. The comparison between perineal plane and mid/low pelvis applications revealed that the latter were associated with greater blood loss, higher proportion of episiotomy, and more frequent use of ultrasound diagnosis. A recent Australian study also highlighted that intrapartum ultrasound is more commonly employed in case of fetal malposition and higher fetal stations, particularly at the mid pelvis. 6 It can be hypothesized that at high fetal stations, visualizing the fetal head position and applying the vacuum are more complex and associated with a higher risk of procedure failure. 1 , 19 Supporting this hypothesis, incorrect placement of the instrument is more frequent when the fetal vertex is at the mid pelvis 20 , and intrapartum ultrasound would aid in the correct positioning of the obstetric vacuum, facilitating a better execution of the procedure. 21 In general, as suggested by international guidelines, knowing the exact position of the fetal head is one of the essential conditions for performing AVB. By promoting increased risk awareness and identifying areas where current practices may be enhanced, we aim to support the development of strategies that foster more efficient and personalized approaches to labor management. This involves not only refining the clinical techniques ensuring enhanced training for healthcare professionals but also integrating insights into patient-specific factors to more effectively tailor interventions, thereby improving overall outcomes for both mothers and neonates. The main strength of the study is the sample size (3833 vaginal deliveries, including 365 AVB) and its clinical and socio-demographic variety. The monocentric design also ensured homogeneous operator expertise and AVB protocols. The limitations of the study include its retrospective nature, the operator-dependent variability in the clinical assessment of fetal station and the absence of adverse events such as vacuum extraction failure. Conclusion In a context where AVB frequently emerges as the safest and most effective therapeutic approach for managing labor dystocia, our study sought to comprehensively address several key aspects. Firstly, we aimed to identify risk factors associated with AVB, which is crucial for counseling and for anticipating and mitigating potential fetal complications. Secondly, we focused on examining both maternal and neonatal outcomes to understand the broader implications of AVB on health and well-being. After adjusting for all factors considered, maternal age (OR 1.04, 95% CI: 1.01-1.07), mode of conception (OR 1.58, 95% CI: 1.07-2.33), epidural analgesia (OR 6.25, 95% CI: 3.05-12.80), gestational age (OR 1.48, 95% CI: 1.31-1.67), and newborn male sex (OR 1.35, 95% CI: 1.06-1.73) were positively associated with AVB, whereas parity (OR 0.20, 95% CI: 0.14-0.29) was inversely associated.Compared to perineal applications, mid/low pelvic applications were associated with greater blood loss (p\u0026lt;0.01), higher episiotomy rate (95.5% vs 88.2%, p=0.03), and increased ultrasound use (62.4% vs 23.5%, p\u0026lt;0.01).\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eConclusions\u003c/strong\u003e: Increased maternal age, nulliparity, medically assisted conception, epidural analgesia, gestational age, and newborn male sex were identified as potential risk factors for AVB. Vacuum extractions performed at mid/low pelvic stations were associated with higher use of episiotomy and greater blood loss.
