Diagnostic value of transvaginal color Doppler ultrasonography for benign lesions of myometrium

Abstract

Objective To analyze the ultrasonographic features and ultrasound manifestations of uterus myoma and adenomyosis diagnosed by transvaginal color Doppler sonography, and to investigate its clinical diagnostic value. Methods Fifty-eight patients suspected with uterus myoma and 50 patients suspected with adenomyosis who were diagnosed and treated in Taiyuan Taihang Hospital from March 2017 to March 2018 were selected as research objects. The morphology, ultrasonic manifestations of the uterus, peak systolic blood flow (PSV), end-diastolic velocity (EDV), pulsatility index (PI), resistance index (RI) of all patients were observed and recorded, and diagnostic value of these indexes was evaluated. Results The coincidence rates of pathological diagnosis of uterus myoma and adenomyosis were 93.10% and 90.00%, respectively (P>0.05). The levels of PI and RI of patients with uterus myoma were lower than those of patients with adenomyosis (P all<0.05). Adler and mass flow signal grades of uterus myoma were mainly graded Ⅰ and Ⅱ. Adler and mass flow signals of adenomyosis were graded 0 and Ⅰ, the difference was statistically significant (P<0.05). The sensitivity of RI and PI in diagnosing uterus myoma was 74.07% and 77.78%, respectively. The specificity of RI and PI in diagnosing adenomyosis was 69.57% and 73.91%, respectively. Conclusions Transvaginal color Doppler ultrasound in the diagnosis of myometrial benign lesions not only can clearly assess morphological changes of uterine mass, but also can dynamically monitor the blood supply status. And transvaginal color Doppler ultrasound combined with PI and RI can enhance the diagnostic sensitivity and specificity, which has a higher diagnostic and differential diagnosis value.
