With improvements in treatment and longer survival, reliable prognostic markers are increasingly important. This study investigated the prognostic value of systemic inflammatory markers and clinicopathological features in RCC patients undergoing nephrectomy. Materials and Methods We retrospectively analyzed 418 patients who underwent radical or partial nephrectomy for RCC between 2009 and 2024. Preoperative hematologic markers, including neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), lymphocyte-to-monocyte ratio (LMR), systemic inflammatory response index (SIRI), and systemic immune-inflammation index (SII), were evaluated. Their associations with overall survival (OS) and recurrence-free survival (RFS) were assessed using survival analyses and multivariate Cox regression. Results The cohort had a median age of 60.5 years and a median follow-up of 72.3 months. Elevated NLR, PLR, SIRI, SII, and reduced LMR were associated with worse OS and RFS. On multivariate analysis, age > 64.5 years, higher tumor grade, NLR ≥ 2.93, and LMR < 3.67 independently predicted poorer OS. Conclusions Simple, routinely available preoperative inflammatory markers, particularly NLR and LMR, may serve as useful predictors of survival in RCC patients. kidney neoplasm tumor biomarker prognostic factors Figures Figure 1 Figure 2 Figure 3 Introduction Renal cell carcinoma (RCC) represents a significant oncological challenge, constituting approximately 3% of all human malignancies and accounting for nearly 90% of cancers originating in the kidney ( 1 , 2 ). Established etiological contributors to RCC include lifestyle factors such as smoking, alongside prevalent comorbidities like hypertension, diabetes, and obesity ( 2 ). The past two decades have witnessed a discernible rise in both the incidence and prevalence of RCC; concurrently, advancements in therapeutic strategies have extended patient follow-up durations, underscoring the need for robust prognostic indicators ( 1 – 4 ). Consequently, there is a heightened demand for reliable markers capable of accurately predicting the disease trajectory ( 3 , 4 ). While traditional prognosticators like tumor grade and stage remain paramount, patient-specific factors including age and existing comorbidities also exert considerable influence on outcomes ( 5 , 6 ). In pursuit of enhanced prognostic accuracy, various models incorporating clinical parameters have been developed. Within this framework, numerous investigations have explored the utility of readily available hematological markers ( 3 , 7 – 16 ). This interest is predicated on the understanding that RCC can induce a systemic inflammatory response within the tumor microenvironment, which subsequently manifests as quantifiable alterations in peripheral blood parameters ( 3 , 7 – 10 ). Specifically, shifts in leukocyte subpopulations, integral components of the host immune surveillance and response, have been correlated with both tumor aggressiveness and patient prognosis ( 3 , 7 – 10 ). To objectively assess these relationships, several hematological ratios have been proposed as potential prognostic tools. Among the most frequently investigated are the neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), lymphocyte-to-monocyte ratio (LMR), systemic inflammatory response index (SIRI), and the systemic immune-inflammation index (SII) ( 3 , 7 – 16 ). Despite their putative prognostic relevance across a spectrum of malignancies, the definitive predictive capacity of these markers in the context of RCC is not yet fully elucidated. Furthermore, a critical challenge remains in the significant heterogeneity of cut-off values proposed for these markers across different studies, precluding the establishment of a standardized consensus ( 3 , 7 – 16 ). Therefore, the present study was designed to meticulously re-evaluate the predictive significance of established clinicopathological features alongside these various inflammatory markers in a cohort of patients who underwent either radical or partial nephrectomy for renal cell carcinoma. Materials and Methods Medical records of patients who underwent partial or radical nephrectomy between January 2009 and October 2024 were reviewed. Among 762 patients who underwent nephrectomy, those with a postoperative pathological diagnosis of renal cell carcinoma (RCC) were included in the study. Patients with metastatic disease at the time of diagnosis or those lost to follow-up were excluded. As a result, data from 418 patients were included in the final analysis Fig. 1 . Demographic data, comorbidities, clinical and pathological characteristics, type of surgery, pathology results, and preoperative laboratory parameters were recorded. The neutrophil-to-lymphocyte ratio (NLR) was calculated by dividing the preoperative neutrophil count by the lymphocyte count. The platelet-to-lymphocyte ratio (PLR) was calculated by dividing the platelet count by the lymphocyte count. The lymphocyte-to-monocyte ratio (LMR) was calculated by dividing the lymphocyte count by the monocyte count. The systemic inflammatory response index (SIRI) was calculated as (neutrophil × monocyte) / lymphocyte, and the systemic immune-inflammation index (SII) was calculated as (neutrophil × platelet) / lymphocyte. Follow-up duration, overall survival (OS), and recurrence-free survival (RFS) were recorded. Oncologic follow-up was mainly performed with contrast-enhanced computed tomography every 6 or 12 months. The study was approved by the local ethic committee. Statistical Analysis: Statistical analyses were performed using SPSS software (version 24.0; SPSS Inc., Armonk, NY). Categorical variables are presented as numbers and percentages, and continuous variables are presented as medians with interquartile ranges (IQR). Differences between groups were analyzed using Fisher’s exact test for categorical variables and the Mann–Whitney U test for continuous variables. Time-to-event outcomes were analyzed using the Kaplan–Meier method, and group differences were assessed with the log-rank test. Univariate Cox proportional hazards regression analyses were conducted to identify factors associated with RFS and OS. Continuous inflammatory markers were categorized using cutoff values determined by receiver operating characteristic (ROC) curve analysis and Youden’s index. Correlations between variables were assessed using Spearman's rank correlation test. Variables significant in univariate analysis were included in multivariate analysis using the Cox proportional hazards model. Hazard ratios (HRs) with corresponding 95% confidence intervals (CIs) were reported. A two-sided p-value < 0.05 was considered statistically significant for all analyses. Results Basic Patient Characteristics: The median age of patients was 60.5 (min:24,max:89) years, of whom 64.8% were male. Most renal tumors (62.4%) were detected incidentally, and the median follow-up time was 72.3 months. Demographic and clinical characteristics are presented in Table 1 . Pathologically, the median tumor diameter was 5 cm (min:1,max:18), clear cell renal cell carcinoma was the most frequent histological subtype (93.1%), and pathological stage T1a was the most common stage (42.1%). Further pathological details are available in Table 2 . The overall survival (OS) rate was 74.4% (Table 2 ). The overall recurrence-free survival (RFS) was 93.8% (Table 2 ). The calculated cut-off values were as follows: NLR: 2.93, LMR: 3.67, PLR: 169.67, SIRI: 1.80, and SII: 1035.46 (Fig. 1 ). The cut-off values for age and tumor size were 64.5 years and 50.5 mm, respectively. Similarly, cut-off values for NLR, LMR, PLR, SIRI, and SII were calculated for the prediction of recurrence-free survival (RFS). These values were as follow: NLR: 3.23, LMR: 2.56, PLR: 183.38, SIRI: 1.81, and SII: 764.05 (Fig. 2 ). Predictive Role of hemotological Markers for OS in Kaplan–Meier Analysis Higher NLR values were associated with a significantly lower mean OS compared to lower NLR values (146 vs. 186 months, respectively; p < 0.001). Conversely, lower LMR values correlated with a significantly shorter mean OS than higher LMR values (130 vs. 152 months, respectively; p < 0.001). Elevated PLR was indicative of a significantly reduced mean OS when contrasted with lower PLR values (104 vs. 153 months, respectively; p < 0.001). Furthermore, a high SIRI corresponded to a significantly shorter mean OS compared to a lower SIRI (132 vs. 143 months, respectively; p < 0.001). An elevated SII was linked to a significantly poorer mean OS, while patients with lower SII values exhibited a mean OS (93 vs. 153 months, respectively; p < 0.001) (Fig. 3 ). Predictive Role of hemotological Markers for RFS in Kaplan–Meier Analysis Evaluation of preoperative hematologic parameters also revealed significant associations with RFS. Higher NLR values were associated with a significantly lower mean RFS compared to lower NLR values (108 vs. 157 months, respectively; p < 0.001). Conversely, lower LMR values correlated with a significantly shorter mean RFS than higher LMR values (132 vs. 187 months, respectively; p < 0.001). Elevated PLR was indicative of a significantly reduced mean RFS when contrasted with lower PLR values (138 vs. 186 months, respectively; p = 0.001). High SIRI corresponded to a significantly shorter mean RFS compared to a lower SIRI (170 vs. 182 months, respectively; p < 0.001). Finally, an elevated SII was linked to a significantly poorer mean RFS, while patients with lower SII values exhibited a mean RFS (172 vs. 182 months, respectively; p < 0.001) (Fig. 3 ). Univariate and Multivariate Analysis of Overall Survival (OS) In univariate analysis, age greater than 64.5 years (p < 0.001), male sex (p = 0.020), tumor size greater than 50.5 mm (p < 0.001), and higher tumor stage (T3 and T4) (p < 0.001 and p = 0.014, respectively) were significantly associated with poorer overall survival (OS) (Table 3 ). Similarly, tumor grade higher than 1 was significantly related to worse OS (p = 0.003 for grade 2, p < 0.001 for grades 3 and 4). However, RCC histological subtype did not have a statistically significant impact on OS (p = 0.566). However, patient age, sex, and RCC histological subtype were not significantly associated with RFS (p = 0.566). Among the hematological parameters, elevated NLR (≥ 3.23, p = 0.001), decreased LMR (< 2.56, p < 0.001), elevated PLR (≥ 183.38, p = 0.013), elevated SIRI (≥ 1.81, p < 0.001), and elevated SII (≥ 764.05, p = 0.001) were significantly associated with worse RFS (Table 5 ). Numerous studies have explored the relationship between systemic inflammatory markers and the prognosis of renal cancers in the literature ( 3 , 7 – 10 ). However, there is no clear consensus regarding the cut-off values and the prognostic impact of these markers ( 3 , 7 – 16 ). Zapala et al. found that an age above 63 years, a tumor size greater than 4 cm, an NLR greater than 2.75, an LMR less than 2, a PLR greater than 170, a SIRI greater than 2.25, and an SII greater than 628 were associated with poorer overall survival (OS) ( 3 ). In contrast, Ishiyama et al. reported that an age above 67 years, a tumor size larger than 7 cm, and an NLR greater than 2.27 were related to worse OS ( 13 ). Elghiaty et al. demonstrated that an age above 56 years and a tumor size greater than 7 cm were associated with poorer OS ( 14 ). In the study by Mei et al., they found that an age above 60 years, a tumor size larger than 5.5 cm, an NLR above 3.99, and a PLR above 158.79 could predict upstaging in cT1 patients ( 15 ). From a different perspective, Gu et al. reported that in patients undergoing cytoreductive nephrectomy, being over 60 years old, having a tumor size greater than 7 cm, an LMR less than 3.23, and a SIRI greater than 1.6 were associated with poorer OS ( 16 ). In our study, we found that elevated NLR, PLR, SIRI, and SII values, as well as decreased LMR values, were associated with worse overall survival. In univariate analysis, all systemic inflammatory markers were found to be predictive of OS, a result that is consistent with the existing literature ( 3 , 7 – 16 ). In multivariate analysis, the most significant predictors of OS were tumor grade, stage, and patient age, in that order. Although elevated NLR and decreased LMR were not as influential as grade, stage, or age, they still provided prognostic information regarding OS. This finding is also in line with previous studies ( 3 , 7 – 16 ). Regarding recurrence-free survival (RFS), similar discrepancies in cut-off values have been observed. Zapala et al. pointed that age above 63 years, a tumor size larger than 4 cm, a PLR greater than 170, a SIRI greater than 2.25, and an SII greater than 628 were associated with shorter RFS, whereas no significant relationship was detected between RFS and NLR or LMR ( 3 ). Ishiyama et al. suggested that an NLR value above 2.28 could be associated with worse RFS ( 13 ). These differences across studies are thought to result from variations in the distribution of pathological stages and differences in patient numbers. Similarly, we found that increased NLR, PLR, SIRI, and SII values and decreased LMR values were associated with poorer RFS, and that all systemic inflammatory markers could be used to predict RFS in univariate analysis. However, due to the limited number of studies on this subject, available evidence is scarce. Zapala et al., for instance, reported that higher PLR and SIRI values were associated with RFS in univariate analysis, but that tumor stage, grade, and size remained the most important parameters, similar to our findings ( 3 ). In multivariate analysis, none of the systemic inflammatory markers significant in univariate analysis could predict RFS ( 3 ). We believe the main reason for this result in our study is the limited number of patients with recurrence. The major limitation of our study is its retrospective design; however there are only a few large single-center studies on systemic inflammatory markers in the literature, which makes our study a valuable contribution. Conclusion While all systemic inflammatory markers appeared to be useful tools for predicting both OS and RFS, we found that NLR and LMR were particularly important for predicting OS, and LMR and SIRI for predicting RFS. However, tumor characteristics and patient age remain the most critical parameters for predicting OS and RFS. Although our study provides preliminary insights into the usability of inflammatory markers, prospective randomized studies are needed for more definitive conclusions. Conversely, lower LMR values correlated with a significantly shorter mean RFS than higher LMR values (132 vs. 187 months, respectively; p\u0026thinsp;\u0026lt;\u0026thinsp;0.001).
