Methods: A cross-sectional analysis was conducted using secondary data from the 9th National Oral Health Survey of Thailand, 2023. The study population included 5,842 individuals aged more than 60 years from 25 provinces in Thailand. Descriptive statistics, Chi-square test and one-way ANOVA were used to assess associations between tooth retention and socio-demographic, behavioral, and clinical factors. Results: Among the participants, 55.3% retained 20 or more natural teeth. Socioeconomic variables, particularly income and age groups, were significantly associated with tooth retention (p < 0.001). Higher income and younger age groups correlated with a greater likelihood of retaining 20 or more teeth. Daily tooth brushing, smoking, and alcohol consumption also showed significant associations. Clinical factors were significantly related to having adequate teeth; high DMFT scores, decayed teeth, cervical tooth wear, and gingival index scores, and root caries were strongly associated with lower tooth retention (p < 0.001). Functional limitations, including oral dryness, chewing, and swallowing difficulties, were additionally linked to poorer tooth loss. Conclusion: The findings highlight the multifactorial nature of tooth retention in later life. Sociodemographic status, health behaviors, and clinical biomarkers play critical roles in maintaining dentition. Integrating oral health into national aging and chronic disease policies, improving access to dental care, and promoting lifelong preventive behaviors are crucial for supporting healthy ageing and functional dentition among older adults in Thailand.
