Abstract
HIS ARTICLE concerns itself with the under-T lying surgical pathology and spread of endometrial adenocarcinoma as determined by clinical, operative, laboratory, and necropsy investigations employed during the initial hospital admission of the patient.Such routine appraisal of the disease is extremely valuable from the standpoint of management and treatment and of the ultimate prognosis.Many studies, including the recent one by Henriksen,s are based on autopsy findings and therefore represent findings in the end stages of the cancer.Such information, while valuable, is not applicable to most patients when first seen, since the cure rate is approximately 75 per cent.Additional notes are included here on the spread to lymph nodes as found by selective lymphadenectomy at the time of surgery and on a proposed classification of endometrial carcinoma.T h e authors have studied the spread of benign and malignant endometrium for several years*, 9 and agree with Sampson that the spread of benign endometrium causing endometriosis is along the same channels employed by malignant endometrium in producing carcinomatosis. MATERIALS AND METHODSMore than 300 consecutive patients have been admitted one or more times to the Woman's Clinic, New York Hospital, with a proved diagnosis of endometrial carcinoma during the period from 1932 to July 1, 1951.While various types of treatment were administered, gross and microscopic examination of the uterus removed at operation was done in 280 cases, and these findings form the basis for the study.In addition, radiographic investigations of lungs, ____
