Abstract

To assess the clinical effect of percutaneous microwave ablation for treatment of diffuse adenomyosis. Fourteen women (mean age, 39.2 years) with symptomatic adenomyosis were treated with PMA under ultrasound guidance. The symptoms included severe dysmenorrheal and anemia. The diagnosis of diffuse adenomyosis was made by magnetic resonance imaging. The two microwave antennas were inserted into the myometrium with adnomyosis for ablation. The microwave emission was seized when the ablated zone was over half of the total lesion. The complications, side effects and early therapeutic effect were observed in the period of treatment and 3 months after treatment. The average ablated time was 900s. The average ablated area was 72% (60%–80%). No patient developed complications in the ablation and after ablation. Thirteen patients had normal menstrual onset in the very month when they received ablation and one case in 30 days. The mount of menstrual bleeding was less than that before treatment. The dysmenorrheal disappeared after treatment in eleven cases, relieved markedly in two and no improvement in one case. The endometrial ectopic growth dose not been observed in all patients after treatment. The clinical effect of percutaneous microwave ablation under ultrasound guidance for treatment of diffuse adenomyosis was positive, and after treatment symptoms such as dysmenorrheal can be eliminated or relieved effectively. The procedure of percutaneous microwave ablation adenomysis is easy to perform and time-saving. It can be an effective modality for adenomysis treatment with uterus un-removed.
