Prevalence of major gynecological symptoms and complications in patients with Ehlers–Danlos Syndrome: a systematic review and meta-Analysis

Abstract

Ehlers-Danlos syndrome is a genetic disorder that affects connective tissue and causes abnormal collagen synthesis, which affects the skin, ligaments, joints, and blood vessels. Several studies corroborate that female patient with this syndrome shows various gynecological symptoms and conditions such as dyspareunia, vulvodynia, dysmenorrhea, and endometriosis. Therefore, the objective of this systematic review and meta-analysis was to determine the prevalence of dyspareunia, dysmenorrhea, and endometriosis in patients diagnosed with Ehlers-Danlos syndrome. This systematic review included seven studies that analyzed patients with gynecological complications and diagnosed with Ehlers-Danlos syndrome. Of these, six were used to measure the combined prevalence of endometriosis and dyspareunia, and only four were used to determine the proportion of dysmenorrhea among patients. In this way, we found a combined prevalence of 78% (95% CI: 65 - 91) with dysmenorrhea, 53% (95% CI: 38 - 68) of patients with dyspareunia and 14% (95% CI: 6 - 23) with endometriosis. We can conclude that dysmenorrhea is the most prevalent gynecological symptom in patients with Ehlers-Danlos syndrome, followed by dyspareunia. Endometriosis is a condition that occurs in 14% of these patients, so its evaluation should be considered in patients diagnosed with Ehlers-Danlos.

El síndrome de Ehlers-Danlos es un trastorno genético que afecta el tejido conectivo y causa una síntesis anormal de colágeno, lo cual afecta la piel, ligamentos, articulaciones y vasos sanguíneos. Estudios indican que pacientes femeninas con este síndrome presentan diferentes síntomas y condiciones ginecológicas como la dispareunia, vulvodinia, dismenorrea y endometriosis. Por este motivo el objetivo de esta revisión sistemática fue conocer cuál es la prevalencia de dispareunia, dismenorrea y endometriosis, en pacientes con diagnosticadas con Síndrome de Ehlers-Danlos. Esta revisión sistemática incluyó 7 estudios que analizaron pacientes con complicaciones ginecológicas y con diagnósticos de Ehlers-Danlos, de los cuales 6 fueron usados para calcular la prevalencia combinada de endometriosis y dispareunia y apenas 4 fueron usados para conocer la proporción de dismenorrea entre las pacientes. En este sentido encontramos una prevalencia combinada de 53% (IC del 95%: 38 - 68) de pacientes con dispareunia, 14% (IC del 95%: 6 - 23) con endometriosis y 78% (IC del 95%: 65 - 91) con dismenorrea. Podemos concluir que la dismenorrea es el síntoma ginecológico más prevalente en pacientes con Síndrome de Ehlers-Danlos, seguido de la dispareunia. La endometriosis es una condición que presenta 14% en estas pacientes por lo que se debe considerar su evaluación en pacientes con diagnóstico de Ehlers-Danlos.
