Expresión de ligandos para las células Natural Killer en el endometrio: comparación entre mujeres afectas de endometriosis y mujeres no afectas

Abstract
Objetivo: Comparar la expresion en endometrio de ligandos para los linfocitos Natural Killer (NK) entre mujeres afectas de endometriosis y mujeres no afectas, concretamente tres ligandos para receptores activadores (ICAM-1, LFA-3, a6b4) y un ligando para el receptor inhibidor (HLA-I). La justificacion para ello es que una alteracion en alguno de estos ligandos conferiria mayor resistencia a este endometrio a la lisis por NK y mayor capacidad de implantacion. Material y metodo: Se obtuvieron muestras mediante legrado de pacientes sometidas a intervencion por varios motivos. Como Grupo Control (GC) se tomaron aquellas pacientes en las que no se habia observado patologia y como Grupo Endometriosis (GE) pacientes en las que la unica patologia observada fue endometriosis. Se estudio la expresion molecular en endometrio congelado mediante inmunohistoquimica (metodo peroxidasa-antiperoxidasa). El estudio se dividio en dos fases: la Fase Piloto, en la que se estudiaron las cuatro moleculas en un maximo de 15 pacientes de cada grupo y la Fase Definitiva, en la que se proceso todo el material disponible si en la Fase Piloto existia algun tipo de diferencia. Resultados: Se recogieron muestras de 76 pacientes, 31 del GC y 45 del GE. Para la molecula LFA-3, en la Fase Piloto, 7 muestras fueron valorables en el GC y 9 en el GE, no observandose diferencias entre ambos grupos (expresion en el 100% de celulas endoteliales, en el 100% de celulas glandulares y en menos del 20% del estroma), no prosiguiendo el estudio para esta molecula. Para la molecula ICAM-1, en la Fase Piloto, 9 muestras fueron valorables en el GC y 9 en el GE, no observando diferencias entre ambos grupos (expresion en el 100% de celulas endoteliales, en el 0% de celulas glandulares y en aisladas celulas del estroma), y tampoco se prosiguio el estudio. Para la molecula a6b4, en la Fase Piloto, si se observaron diferencias: en las celulas glandulares, 9/10 muestras expresaban la molecula polarizada y 1/10 despolarizada en el GC, mientras que 5/13 la expresaban polarizada y 8/13 despolarizada en el grupo Endometriosis (diferencia estadisticamente significativa, test exacto de Fisher p=0.0376). Se decidio pasar a la Fase Definitiva y se confirmaron las diferencias: en el GC 16/19 muestras valorables presentaban expresion polarizada mientras que en el GE eran 10/30 (Chi2 p=0.001). Para HLA I, en la Fase Piloto, 9 muestras fueron valorables en el GC y 10 en el GE. Se observo una expresion de HLA en un mayor porcentaje de celulas glandulares en el GE (mediana 100) comparando con el GC (mediana 72), aunque la diferencia no fue significativa. Se observo un mayor porcentaje de expresion en el estroma en el GE (medianas 72 vs 20) y la diferencia si fue significativa (U de Mannwhitney p=0.043). Se decidio pasar a la Fase Definitiva, obteniendo 15 muestras valorables en el GC y 25 en el GE. La expresion de HLA I se producia en un mayor porcentaje de celulas en el GE, siendo las diferencias significativas tanto para glandulas (mediana 100 vs 80, U de Mannwhitney p=0.043) como para estroma (mediana 60 vs 20, U de Mannwhitney p=0.003). Conclusion: No existen diferencias en la expresion endometrial de LFA-3 e ICAM-1 entre GC y GE. Existen diferencias significativas en cuanto al patron de expresion de a6b4 en las glandulas entre ambos grupos (mayor porcentaje de expresion despolarizada en el GE). Existe un porcentaje significativamente mayor de celulas positivas para HLA I en el GE frente al GC, tanto en glandulas como en estroma.
Objective: to compare the expression in endometrium of certain Natural Killer receptors, between women with and women without endometriosis. We studied expression of three molecules binding to activating receptors (LFA-3, ICAM_1 and a6b4) and the expression of HLA-I, binding to inhibiting receptors. The alteration of any of these molecules would confer endometrial resistance to NK lysis. Material and method: endometrial samples were obtained by curettage from patients operated for several causes. As Control Group (CG), we took patients where no genital disease was observed and as Endometriosis Group (EG)we took patients where the only disease was endometriosis. Frozen endometrium was used for immunohistochemical study with the peroxidase-antiperoxidase method. There were two parts in the study: the Previous Study, where 15 patients in each group were studied for all the molecules, and the Definitive Study, where all the patients where studied for a given molecule if in the Previous Study any difference was suggested. Results: endometrial samples were taken in 76 patients, 31 in the CG and 45 in the EG. For LFA-3, 7 samples in the CG and 9 in the EG were valuable in the Previous Study. There were no differences between both groups ( expression in all the endothelial and glandular cells and in 20% of stromal cells) so we didnt proceed with the Definitive Study. For ICAM-1, 9 samples were valuable in the CG and 9 in the EG in the Previous Study. There were no differences between the two groups ( expression in all the endothelial cells, in none of the glandular cells and in scattered cells of the stroma), so we didnt proceed with the definitive study neither. For a6b4, in the Previous Study we observed important differences: 9/10 samples showed polarized expression and 1/10 depolarized expression in glandular cells in the CG, but 5/13 shwed polarized expression and 8/13 depolarized expression in the EG (Fisher exact test p=0.0376). The results were confirmed in the Definitive Study: 16/19 showed polarized expression in the CG but 10/30 in the EG (Chi2 p= 0.001).For HLA-I, 9 samples were valuable in the CG and 10 in the EG in the Previous Study. The EG showed a higher percentage of positive cells than the CG, but the difference was no significant for glands (median 100 vs 72) and it was significant for stroma (median 72 vs 20, Mannwhitney U p=0.043). The results were confirmed for both glands and stroma in the Definitive Study: the percentage of positive cells was significantly higher in the EG (N=25) than in the CG (N=15), both for glands (median 100 vs 80, Mannwhitney U p=0.003) and for stroma (median 60 vs 20, Mannwhitney U p= 0.003). Conclusion: there were no differences between both groups in the expression of LFA-3 and ICAM-1. The EG showed a significantly higher number of samples expressing a depolarized pattern of a6b4 than the CG. Samples in the EG showed a significantly higher percentage of positive cells for HLA I than in the CG.
