While benign HPB conditions are often curable, malignant forms, particularly hepatocellular carcinoma and pancreatic cancer, carry poor prognoses, exacerbated by late presentation and limited health system capacity. Methods We conducted a one-year prospective cohort study (May 2024–May 2025) at a tertiary hospital in Tanzania, enrolling patients aged ≥13 years with surgically managed HPB diseases. Analysis was done using SPSS version 25. Results A total of 160 patients were included. Benign diseases accounted for 58.8%, predominantly gallbladder pathologies (74.9%), with a mortality rate of 4.3%. Malignant diseases comprised 41.3%, most commonly liver cancer (31.4%) and pancreatic cancer (28.4%). Overall mortality was 13.1% (n=21), of which 85.7% occurred in malignant cases. Mortality was highest in liver cancer (71.4%) and pancreatic cancer (26.3%). Independent predictors of mortality included age >60 years (AOR 2.50, 95% CI 1.10–5.80, p=0.02), operative time ≥4 hours (AOR 2.20, 95% CI 1.05–4.65, p=0.03), and digestive bypass procedures (AOR 2.20, 95% CI 1.00–4.95, p=0.05). Conclusion HPB diseases pose a substantial burden in Tanzania, with malignant conditions carrying particularly high mortality. Advanced age, prolonged operative duration, and palliative procedures independently predicted death. These findings highlight the urgent need for earlier diagnosis, structured referral systems, and expanded capacity for safe HPB resections. Strengthening multidisciplinary care and regional training in hepatobiliary and pancreatic surgery is critical to improve outcomes in resource-limited settings. Hepatopancreatobiliary diseases Surgical outcomes Malignancy Tanzania Figures Figure 1 Introduction Hepatic, pancreatic, and biliary (HPB) diseases encompass a wide spectrum of benign and malignant conditions that significantly contribute to global surgical morbidity and mortality. Benign entities such as choledocholithiasis, hepatic cysts, and pancreatitis arise from obstructive, inflammatory, or congenital processes, while malignant tumors, particularly hepatocellular carcinoma, cholangiocarcinoma, and pancreatic adenocarcinoma are driven by genetic, environmental, and viral factors with distinct morphological and histopathological patterns ( 1 , 2 ). The natural history of malignant diseases diverges markedly, exhibiting typically aggressive progression, vascular invasion, and a dismal prognosis ( 3 , 4 ). Clinically, HPB disorders manifest with nonspecific but often debilitating symptoms such as jaundice, abdominal pain, weight loss, or gastrointestinal bleeding. Advances in diagnostic imaging (CT, MRI, MRCP, endoscopic ultrasound) have improved detection, while surgical strategies ranging from resections and bypass procedures to minimally invasive and transplant approaches remain central to curative intent ( 5 – 8 ). Despite these advances, outcomes remain heterogeneous: benign conditions are often resolved with timely intervention, whereas malignant HPB diseases carry high recurrence rates and limited long-term survival ( 9 ). Globally, HPB malignancies are among the leading causes of cancer-related deaths. Hepatocellular carcinoma ranks as the sixth most common cancer worldwide, with marked prevalence in sub-Saharan Africa and Asia due to chronic viral hepatitis and aflatoxin exposure ( 5 ). Pancreatic cancer, though less common, has one of the lowest survival rates, with a global 5-year survival below 10% ( 10 ). In Tanzania and other low- and middle-income countries (LMICs), the burden of HPB diseases is compounded by late presentation, delays in referral, inadequate diagnostic facilities, and limited surgical capacity. Existing reports from tertiary hospitals highlight high proportions of advancedstage disease and reliance on palliative interventions, but comprehensive analyses of disease spectrum, surgical approaches, and outcomes remain scarce ( 11 – 14 ). At Bugando Medical Centre (BMC), one of the country’s largest tertiary referral hospitals, many patients with HPB diseases present at advanced stages where curative surgery is no longer feasible. This underscores an urgent need for local data to guide surgical decision-making, resource allocation, and policy development. This study therefore aimed to determine the clinical profile, management, and short-term outcomes of patients with hepatobiliary diseases at a tertiary Tanzanian center. Materials and methods This study employed a prospective observational hospital-based cohort design conducted at a tertiary care facility in Tanzania, spanning a one-year period from May 2024 to May 2025. The study population comprised patients aged 13 years and older, presenting with hepatopancreatobiliary (HPB) disorders, encompassing both benign and malignant conditions who underwent surgical intervention within the General Surgery department. Patients managed exclusively outside the hospital, those with incomplete medical records, or individuals unwilling to provide consent for follow-up were excluded from the study. Participants were enrolled consecutively until the requisite sample size was attained. Independent variables included demographic characteristics, clinical and pathological features, diagnostic modalities, and therapeutic approaches, while dependent variables concentrated on perioperative and follow-up outcomes such as morbidity, mortality, duration of hospital stay, and the necessity for reoperation. Statistical analysis The data were subjected to a descriptive statistical analysis using SPSS version 27. Variables with p < 0.2 in bivariate analysis were entered into multivariate logistic regression. A p-value < 0.05 was considered statistically significant. Results A total of 184 patients presenting with hepatic, pancreatic, and biliary pathologies were initially enrolled at Bugando Medical Center, of whom 24 were excluded due to incomplete records, non-operative management, loss to follow-up, or repeat admissions. The final analytic cohort comprised 160 surgically managed patients who met the inclusion criteria. The median age was 50 years (SD ±15.8), spanning a wide age range from adolescence to senescence. Socio-demographic and clinical characteristics The predominant age group was 19–45 years (41.9%), followed by those aged above 61 years (30.6%) and 46–60 years (23.8%), while adolescents (13–18 years) constituted only 3.8%. Females represented a slightly greater proportion (55.0%) than males (45.0%). Geographically, 44.4% of patients resided in Mwanza, whereas others were referred from Musoma (18.1%), Geita (16.3%), Shinyanga (11.3%), and other distant regions (10.0%). Abdominal pain was the most prevalent presenting symptom (88.1%), followed by jaundice (45.6%), pruritus (38.8%), nausea and vomiting (37.5%), weight loss (31.3%), and palpable abdominal masses (28.1%); fever was reported infrequently (7.5%). On examination, abdominal tenderness was the most common finding (63.7%), while palpable masses were detected in 27.5%, ascites in 5.6%, and miscellaneous findings in 3.1%. Comorbidities were present in 40.0% of patients, with hypertension (21.9%), diabetes mellitus (10.0%), and HIV infection (8.1%) being the most frequent. The remaining 60.0% had no underlying chronic illness. Hematologic evaluation revealed that 54.4% had normal hemoglobin levels (≥11.5 g/dL), while 35.0% exhibited moderate and 10.6% severe anemia. Ultrasonography was the primary diagnostic modality (49.4%), followed by CT scan (35.0%) and MRI (14.4%), with only 1.3% lacking imaging assessment. Overall, benign diseases predominated (58.8%), whereas malignant pathologies accounted for 41.3% of cases [Table 1]. All patients aged 13–18 years had benign conditions, with no malignancies recorded. In the 19–45 year group, benign lesions were still predominant (86.6%), while malignant cases accounted for only 13.4%, reflecting the rarity of hepatopancreatobiliary cancers in younger populations. From mid-adulthood onwards, the balance shifted significantly. In the 46–60 year group, malignant diseases nearly matched benign cases (47.4%), highlighting a transitional phase. The highest burden was seen in patients over 60 years, where malignancies dominated (79.6%) and benign cases were relatively few (20.4%) [Figure 1]. Perioperative patterns The median age of patients with benign conditions was 42 years, while malignant cases had a significantly higher median age of 62 years (p=0.0001), indicating that malignancy predominantly affects patients over 60, whereas benign disease is more common in those under 45. Females were more likely to present with benign disease (72.7%), and males were more frequently affected by malignancy (58.3%), though this difference was not statistically significant (p=0.097). Gallbladder diseases dominated the benign group (86.0%), whereas liver (83.3%) and pancreatic diseases (75.0%) were mostly malignant, with bile duct diseases evenly distributed (50% each). Surgical parameters including surgery urgency, operative duration, and ICU admission, did not differ significantly between groups, although longer surgeries (≥4 hours) were mostly associated with malignancies (76.7%), reflecting surgical procedure complexity. On postoperative outcomes, most patients were discharged; however, mortality was significantly higher in malignant cases (85.7% of deaths; p=0.0001), highlighting the prognostic impact of malignancy. Among benign disease types, cholelithiasis/cholecystitis was most frequent (74.9%), followed by choledocholithiasis (9.6%), pancreatic cysts (6.4%), and hepatic cysts or abscesses (8.8%). Malignant cases were led by liver cancers (31.4%) and pancreatic cancers (28.4%), with cholangiocarcinomas (13.2%), periampullary tumors (9.1%), and gallbladder cancers (5.0%) comprising the remainder. Surgical interventions reflected this distribution: cholecystectomies (46.3%) were most common, followed by digestive bypass procedures (20.6%), hepatectomies (8.8%), cholecystectomy with CBD exploration (8.1%), open biopsies (4.4%), and other surgeries (11.9%) [Tables 2]. Deaths were rare in benign conditions; cholelithiasis and choledocholithiasis had minimal or no mortality, whereas hepatic abscesses (28.6%) and other severe infections showed higher risk. Among malignancies, liver cancer was the deadliest (71.4%), followed by pancreatic (26.3%) and gallbladder cancers (25%), while cholangiocarcinoma (22.2%) and periampullary tumors (16.7%) fared better due to palliative bypass procedures reducing operative risk. Mortality strongly correlated with surgical complexity: cholecystectomy (4.1%) and CBD exploration (7.7%) were safest, digestive bypasses (21.2%) carried moderate risk, and hepatectomy (71.4%; p=0.004) had the highest fatality, underscoring the impact of advanced disease and operative difficulty [Table 5]. Patients above 60 years had significantly higher odds of death (AOR 2.50, 95% CI: 1.10–5.80, p =0.02), emphasizing the vulnerability of older patients with reduced physiological reserve. Similarly, surgeries lasting ≥4 hours were associated with increased mortality (AOR 2.20, 95% CI: 1.05–4.65, p =0.03), reflecting the risks linked to surgical complexity and prolonged operative stress. Malignant diseases were strongly linked to poorer outcomes compared to benign conditions. Pancreatic cancers (AOR 2.90, 95% CI: 1.25–6.80, p =0.01) and liver cancers (AOR 2.40, 95% CI: 1.05–5.50, p =0.04) independently predicted mortality, while gallbladder cancers and cholangiocarcinomas showed borderline significance. These findings highlight the aggressive nature and late presentation of malignancies in our setting. In contrast, benign conditions such as choledocholithiasis, hepatic cysts, or abscesses did not significantly increase mortality after adjustment, supporting their favorable prognosis when surgically managed. The type of surgery also influenced outcomes. Digestive bypass procedures, usually performed for advanced or unresectable malignancies, carried higher odds of mortality (AOR 2.20, 95% CI: 1.00–4.95, p =0.05). Standard procedures like cholecystectomy and hepatectomy were not independent predictors of death, suggesting relative safety in selected patients. Insurance status showed an initial crude association but lost significance after adjustment, likely due to confounding [Table 6] . Our findings closely reflect patterns observed at Tanzanian and nearby sub-Saharan centers. Benign HPB diseases such as gallbladder pathologies, choledocholithiasis, and hepatic abscesses were largely curable with timely surgery, showing minimal mortality, particularly with cholecystectomy as the safest procedure. On the other hand, malignant HPB diseases predominated in older patients (median 62 years) and carried high mortality rates; 71.4% for liver cancer and 26.3% for pancreatic cancer. Similar late presentation and poor surgical outcomes have been documented in obstructive jaundice and hepatocellular carcinoma at other tertiary Tanzanian hospital centers ( 9 , 11 , 12 ). In Kenya and West Africa, pancreatic tumors are often unresectable, leading to palliative procedures ( 2 , 6 ). The African Surgical Outcomes Study (ASOS) also highlights elevated perioperative mortality in major abdominal surgery across Africa, which aligns with our hepatectomy results ( 15 ). Comparison with Global (Asia, in contrast to high-income countries, our mortality rates, particularly after hepatectomy (71.4%), far exceed the typical 3–10% mortality reported in Europe and North America under optimized perioperative systems ( 16 – 18 ). Digestive bypass procedures are also far more lethal in our setting (21.2%) compared to settings with enhanced recovery protocols ( 6 ). Similar patterns but to a lesser extent have been reported in LMICs across Asia, where late presentation and limited infrastructure similarly challenge HPB surgery outcomes ( 8 , 15 , 17 ). The most highlighted association between increasing age and malignant hepatopancreatobiliary (HPB) pathology underscores the critical need for early detection and expedited referral pathways for older patients presenting with HPB red-flag signs such as jaundice, weight loss, and the presence of an intra-abdominal mass, potentially shifting disease toward a more resectable stage and improving outcomes ( 19 ). Our mortality rate (24.3%) mirrors findings from Kenya and Nigeria (20–30%), underscoring the persistent challenge of late presentation and limited perioperative support that continue to drive poor surgical outcomes across sub-Saharan Africa ( 2 , 5 , 8 ). Implementation of focused perioperative strategies; such as rehabilitation, specialist anesthetic planning, and coordination with ICU services for individuals who are over 60, undergoing operations of more than 4 hours, or diagnosed with malignancy can substantially reduce perioperative mortality ( 9 , 16 , 20 ). The exceptionally high hepatectomy mortality in this cohort highlights an urgent need for surgical strategy reassessment, including deliberate case selection, neoadjuvant therapy, or staged approaches until perioperative resources are bolstered ( 5 , 7 , 18 ). Furthermore, the establishment of multidisciplinary HPB teams, adoption of structured perioperative protocols, and adaptation of enhanced recovery frameworks to local settings may significantly mitigate operative risk ( 6 ). Finally, creating regional registries and research networks, modeled after the successful African Surgical Outcomes Study (ASOS), will facilitate outcome tracking, quality benchmarking, and data-driven improvements in HPB surgical care. Study Limitations The study’s retrospective single-center design inherently limits generalizability to other settings. Also, missing or incomplete patient records and variable follow-up durations may have introduced bias in outcome assessment. In addition, small sample sizes within some subgroups limited statistical power, potentially underestimating associations. Finally, reliance on chart documentation may not fully capture clinical decision-making nuances or unrecorded perioperative factors. Conclusion Mortality following hepatopancreatobiliary surgeries was markedly higher in malignant compared to benign diseases, with pancreatic and liver cancers showing the worst outcomes. Older age and longer operative duration emerged as key determinants of poor prognosis, reflecting the need for meticulous patient selection and perioperative planning. These findings call for strengthening early diagnostic pathways, optimizing perioperative care protocols, and prioritizing high-risk patients for intensive monitoring. Future research should expand to multicenter and longitudinal designs to capture survival trends and identify novel predictors that can refine surgical decision-making and improve outcomes in this vulnerable population. The highest burden was seen in patients over 60 years, where malignancies dominated (79.6%) and benign cases were relatively few (20.4%) \u003cstrong\u003e[Figure 1].\u003c/strong\u003e\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003ePerioperative patterns\u003c/strong\u003e\u003c/p\u003e\n\u003cp\u003eThe median age of patients with benign conditions was 42 years, while malignant cases had a significantly higher median age of 62 years (p=0.0001), indicating that malignancy predominantly affects patients over 60, whereas benign disease is more common in those under 45. Females were more likely to present with benign disease (72.7%), and males were more frequently affected by malignancy (58.3%), though this difference was not statistically significant (p=0.097). Gallbladder diseases dominated the benign group (86.0%), whereas liver (83.3%) and pancreatic diseases (75.0%) were mostly malignant, with bile duct diseases evenly distributed (50% each). Surgical parameters including surgery urgency, operative duration, and ICU admission, did not differ significantly between groups, although longer surgeries (\u0026ge;4 hours) were mostly associated with malignancies (76.7%), reflecting surgical procedure complexity. On postoperative outcomes, most patients were discharged; however, mortality was significantly higher in malignant cases (85.7% of deaths; p=0.0001), highlighting the prognostic impact of malignancy.\u003c/p\u003e\n\u003cp\u003eAmong benign disease types, cholelithiasis/cholecystitis was most frequent (74.9%), followed by choledocholithiasis (9.6%), pancreatic cysts (6.4%), and hepatic cysts or abscesses (8.8%). Malignant cases were led by liver cancers (31.4%) and pancreatic cancers (28.4%), with cholangiocarcinomas (13.2%), periampullary tumors (9.1%), and gallbladder cancers (5.0%) comprising the remainder. Surgical interventions reflected this distribution: cholecystectomies (46.3%) were most common, followed by digestive bypass procedures (20.6%), hepatectomies (8.8%), cholecystectomy with CBD
