This effect has been felt profoundly in New York City, one of the earliest COVID-19 hotspots, even as restrictions have eased. The goal of this study was to characterize sport participation rates in New York City prior to and following the onset of the COVID-19 pandemic. This cross-sectional study utilized a retrospective survey on children aged 6-17 with musculoskeletal complaints, assessing sports participation before and after the pandemic. Participants included parents of 242 children (123 males, 119 females) with an average age of 12.5 years ±3.2 years. Even 18 months into the COVID-19 pandemic, significant decreases were observed in gym class (p<0.001), pick-up games/free play (p=0.0027), basketball (p=0.002), and soccer (p<0.0001) in New York City's children, with a pronounced impact on female adolescents in gym class and pick-up level games (p<0.05). Efforts to reverse the decline in sports participation among children post-pandemic are crucial for their overall well-being. These interventions can contribute to lifelong benefits, necessitating attention in clinical settings and public health initiatives. Figures Figure 1 Figure 2 Figure 3 Figure 4 INTRODUCTION Engaging in physical activities is important for preserving health, especially in children who are rapidly developing physically, socially, and cognitively. However, in 2016, 81.0% of adolescents aged 11 and 17 worldwide failed to meet 60-minute daily physical activity of moderate-to-vigorous intensity recommended by WHO. 1 The decline of sports participation in adolescents has been well documented. 2 A reported 35% of youth athletes in North America quit participation in sports annually, with two-thirds of participants being between 7 and 18 years. 3 – 5 Among U.S. adolescents, the sports attrition rate was 32% between 8th and 12th grade, where female students were affected two to three times more than male students in all sports. 6 Some previously identified factors contributing to dropout are gender differences, external influence, time constraints, negative sports experience, and lack of access. 7 – 9 When further examining gender differences, it is found that female adolescents participate in substantially fewer sports than their male counterparts. 8 One recent study demonstrated that a lack of proper breast support hinders sports participation for 27% of young women. 4 Specifically, mid-range adolescents between 15 and 18 years more frequently reported a lack of breast support and discomfort as preventing them from being physically active or engaging in exercise. 10 Recently, the COVID-19 pandemic has further contributed to a decline in sports due to temporary shelter-in-place orders, social distancing policies, strict-return-to-play protocols, and COVID-19-specific sports training, profoundly impacting how adolescents engage in organized sports. 11 Among the cities greatly impacted, New York City was hit the earliest, hardest, and longest, making it one of the first epicenters in the United States. As the COVID-19 hotspot, the city closed over 344 city schools at one point. 12 A reported 2.9 million people in New York City alone have tested positive for COVID-19, with about 40,000 deaths reported, which each borough impacted differently. 13 As of November 2022, the number of positive cases has been reported in each of the boroughs as follows: 859,491 in Brooklyn, 812,322 in Queens, 553,028 in Manhattan, 479,618 in the Bronx, and 200,443 in Staten Island. 13 As many children and families seek to re-engage with sport, we sought to characterize sports participation and attrition before and after the COVID-19 pandemic. This study aimed to characterize sports participation rates in New York City before and after the onset of the COVID-19 pandemic. The primary hypothesis was that there would be a decline in sports participation rates for all children and adolescents after the pandemic’s onset. We additionally hypothesized that females would have higher rates of sports attrition than their male counterparts. METHODS Data for this study were obtained from sports participation surveys distributed to parents of children between 6 and 17 years who were evaluated at one of four pediatric orthopedic surgery clinics in New York City from October 2021 to April 2022. The age range includes preadolescents and adolescents who may participate in formal sports. This study was approved by the Institutional Review Board at NYU Langone Health, ensuring compliance with research ethic guidelines and safe guarding patient confidentiality. We advertised the study using an IRB-approved recruitment flyer to be posted in the pediatric orthopedic surgery clinics when the PI and sub-I were in the clinic. Recruiters went into appropriate patient examination rooms and explained the research study while patients were waiting to be seen by the PI or the sub-I. Then, participants were asked to complete a one-time anonymous survey online via REDCap. Upon scanning the QR code on the flyer, participants were directed to REDCap on their mobile devices to read the study information and move on to survey questions once they consented to voluntarily participate in the study. The survey included 12 questions with an approximate completion time of 5 minutes. The participants were asked about their child's age, gender, borough, specific type of sports played, and the number of hours played per week before, during, and after the COVID-19 pandemic. We used descriptive statistics to analyze patient demographics and physical activity levels. We employed McNemar’s comparison test to determine whether any specific type of sports participation or attrition was significantly associated with gender and age. We performed analysis using SAS, and statistical significance was set at a p-value < 0.05. RESULTS From October 2021 to April 2022, 242 parents of children between 6 and 17 completed the survey. The mean age of the children was 12.45 years (± 3.23, range 6–17 years). Of those children, 123 (50.83%) were males, 119 (49.17%) were females, 137 (56.61%) were below 14 years, and 105 (43.39%) were 14 years and above. The mean number of hours these children played sports per week was 7.23 hours (± 6.3, range 0–45 hours) and 7.25 hours (± 6.2, range 0–35 hours) before and after COVID-19, respectively. Table 1 details the distribution of sports participation across gender before and after COVID-19. The parents could multi-select the nature of their child’s physical activities, including gym class, school competitive, club non-competitive, club competitive, pick-up games, none of the above, and others. Additionally, they listed specific sports their child played, such as basketball, football, gymnastics, hockey, karate, skateboarding, skiing, soccer, swimming, volleyball, track and field, and others. Among the categories, children’s changes in participation in gym class ( P = 0.0007), pick-up games ( P = 0.0027), basketball ( P = 0.0020), soccer ( P < 0.0001), and track and field ( P = 0.0290) were demonstrated to be statistically significant (Table 2 ). We found significant differences in the five categories: gym class is mandated at school, pick-up games are free play, and basketball, soccer, track and field are three types of organized sports. After the pandemic, 32 (13.22%) children ceased participating in gym class, whereas 10 (4.13%) children began participating. 27 (11.16%) children ceased to participate in pick-up games, whereas 9 (3.72%) children began playing pick-up games. 26 (10.74%) children ceased to play basketball, whereas 8 (3.31%) children began playing basketball. 23 (9.50%) children stopped playing soccer, whereas 3 (1.24%) children began playing. 4 (1.65%) children ceased to play track and field, whereas 13 (5.37%) children began playing track and field (Fig. 1 ). Only female children contributed to the significant change in gym class participation ( P < 0.0001) (Fig. 2 ). Both male and female children contributed equally to the significant change in pick-up game participation ( P = 0.0290). Both male and female children contributed with varying impacts to the significant change in basketball participation ( P < 0.0001 and P = 0.0020, respectively). Both male and female children contributed with varying impacts to the significant change in soccer participation ( P = 0.0075 and P = 0.0039, respectively). Only female children contributed to the significant change in track and field participation ( P = 0.0027). We divided children into preadolescent and younger adolescent (6–13 years) and older (14–17 years) groups. In Figs. 3 and 4 , only older adolescents contributed to the significant change in gym class, pick-up game, and basketball participation ( P = 0.0076, P = 0.0114, and P = 0.0009, respectively). Both preadolescents and adolescents contributed with varying impact to the significant change in soccer participation ( P = 0.0201 and P = 0.0027, respectively). However, age did not significantly affect track and field participation. Table 4 Sports Participation by Age Before and After Covid-19 Age Before COVID-19 After COVID-19 p-value No Yes Gym class < 14 No 18 7 0.0881 Yes 15 97 ≥ 14 No 11 3 0.0076* Yes 14 52 Pick-up games < 14 No 61 8 0.2008 Yes 14 54 ≥ 14 No 38 1 0.0114* Yes 9 32 Basketball < 14 No 75 8 0.3711 Yes 12 42 ≥ 14 No 45 0 0.0009* Yes 11 24 Soccer < 14 No 90 3 0.0201* Yes 12 32 ≥ 14 No 58 0 0.0027* Yes 9 13 Track and field < 14 No 122 8 0.0578 Yes 2 5 ≥ 14 No 73 1 0.0564 Yes 2 4 DISCUSSION Children’s sports participation leads to countless health benefits, including improved bone density, increased physical fitness, reduced cardiovascular and metabolic disease risk, strengthened immune system, and reduced symptoms of depression and anxiety. 14 , 15 Early sports sampling in children is reported to have a positive correlation with their continued participation in sports during their adolescent years. 3 Additionally, the adolescent years are a critical period for physical, social, and cognitive development, during which health behaviors, including physical activities, stabilize and continue into adulthood. 16 , 17 Although early physical activity is highly recommended, dropout rates in sports are most substantial in adolescents resulting in insufficient physical activity levels worldwide. 1 , 4 Some major barriers for adolescents engaging in physical activity are school-related, including too much schoolwork, lack of time after schoolwork, and lost opportunities to participate in school sports. 18 On top of these academic-related reasons, the recent lockdown, social distancing, and organized sports being canceled due to the COVID-19 pandemic further exacerbated adolescents’ lost opportunities to participate in physical activity. 18 Analysis of children’s physical activity levels before and after the pandemic demonstrates that more females ceased participating in gym class than males and more older adolescents than preadolescents or younger adolescents. Notably, children’s track and field involvement significantly increased after the pandemic, contrary to the decline in the rest of the categories. Increased track and field participation seen in our data might be due to the nature of the sport being outdoor and having individual events not necessarily requiring organized team efforts. Our findings are consistent with recent studies on female adolescents’ decline in physical activity levels post-pandemic. Male students were reported to have higher sports participation and wellness scores than female students while female students were less likely to maintain similar sports activity levels than male students during lockdown. 18 , 19 highschool students (16–17 years) were reported to have lower sports participation and wellness scores than middle school students (13–15 years). 19 Another recent post-pandemic study reported a decline across all outdoor activity participation for American adolescents by 21.6% during COVID-19 and emphasized the importance of time outdoors to bolster mental health and resilience among adolescents and combat COVID-19-induced stressors. 20 There are limitations to this study. Our small sample size limited the strength of associations between sports participation level and different demographic characteristics of this cohort, such as specific boroughs or racial/ethnical backgrounds. This retrospective review is subject to recall bias that influences the accuracy of participants’ answers to the survey. This study was also limited to parents of children who visited pediatric orthopedic surgery clinics, and results may not be generalizable to all children in New York City. CONCLUSION Our study demonstrated that female adolescents are associated with higher sports attrition among children in New York City. This finding contributes additional data and supports other studies demonstrating that female adolescents are more vulnerable to leaving sports. Compared to before the COVID-19 pandemic, adolescents have a significant sports attrition rate in post-pandemic New York City. Understanding the patterns in post-pandemic sports attrition may provide insight into developing methods to encourage children to return to playing sports. This effect has been felt profoundly in New York City, one of the earliest COVID-19 hotspots, even as restrictions have eased. The goal of this study was to characterize sport participation rates in New York City prior to and following the onset of the COVID-19 pandemic. This cross-sectional study utilized a retrospective survey on children aged 6-17 with musculoskeletal complaints, assessing sports participation before and after the pandemic. Participants included parents of 242 children (123 males, 119 females) with an average age of 12.5 years ±3.2 years. Even 18 months into the COVID-19 pandemic, significant decreases were observed in gym class (p\u003c0.001), pick-up games/free play (p=0.0027), basketball (p=0.002), and soccer (p\u003c0.0001) in New York City's children, with a pronounced impact on female adolescents in gym class and pick-up level games (p\u003c0.05). Efforts to reverse the decline in sports participation among children post-pandemic are crucial for their overall well-being. These interventions can contribute to lifelong benefits, necessitating attention in clinical settings and public health initiatives.
