Does Anatomical Defects in Pelvic Floor Supportive Structures Associate with Overactive Bladder: A Cross-Sectional Study

Abstract

Objective: investigate the correlation between POP location and OAB Design: Single-center, cross-sectional study Setting: Peking University People’s Hospital (PKUPH) Population: A total of 550 women who visited PKUPH and underwent surgery due to stage III-IV POP (POP-Q staging) were included in this study. Methods: : In this cross-sectional study, women who underwent surgeries because of stage III-IV POP were included. We collected their basic information, occurrence of OAB-related symptoms, data of pelvic floor ultrasound and parameters of urodynamic test. Binary logistic regression model was used to explore the correlation between POP location and OAB. Results: : A total of 550 women were included in analysis. Three hundred and twenty-six women (59.27%) reported at least one OAB-related symptom evaluated by OABSS. The prevalence of OAB increased with advanced apical prolapse. The odds for a higher prevalence of UUI increased by 17.2% (95%CI 1.047-1.312) for D point per centimeter of additional descent. In patients with severe POP, there was no significant difference in the DWT and DO between the OAB and non-OAB groups. Conclusions: : The descent of D point was associated with higher risk of urgency and UUI in women with severe POP, providing evidence for the hypothesis of integral theory. It indicated that apical repair was essential to improve the OAB in women with severe POP.
