This descriptive study was carried out to determine whether headaches experienced by individuals with migraine are influenced by climate changes. Methods The data were collected from migraine patients visiting the neurology outpatient clinic of a training and research hospital. The data collection tools included a Personal Information Form, Climate Effects Form, Visual Analog Scale (VAS), and Headache Impact Test (HIT). The sample consisted of 153 migraine patients. Results The participants had a mean age of 37.10 ± 9.79 years, and the average duration of migraine diagnosis was 8.17 ± 8.15 years. Of the participants, 83.7% were female, 32.0% had a bachelor’s degree, 75.8% were single, 77.1% had a middle income level, and 56.2% were employed. In the study, participants most frequently reported that increases in temperature (71.9%), seasonal transitions (71.2%), and wind (68.0%) exacerbated their headaches. However, 11.8% of participants reported cold weather to alleviate headaches, and 66.0% reported rain to have no effect. Conclusions In conclusion, the results suggest that headaches in individuals with migraine can be influenced by climate changes. These results highlight the importance of considering climatic factors in migraine management and underscore the need for developing individualized preventive strategies. Pain Headache Migraine Weather Climate Introduction Migraine is a common cause of disability and loss of work. Migraine attacks have a complex nature and are recurrent neurological events that can last hours to days and significantly affect daily activities and the quality of life [ 1 ]. Migraine is a health condition that incorporates more than just a severe headache. Its symptoms vary among individuals, often including recurring headaches [ 2 ]. During acute and chronic migraine episodes, a range of symptoms may accompany severe pain, including sensitivity to light, sound, and smell, appetite loss, fatigue, dizziness, visual problems (such as double vision, vision loss, or blurred vision), nausea and/or vomiting, numbness, hearing reduction, imbalance while walking, weight loss, sleep disturbances, and diarrhea [ 3 ]. Migraine attacks may last for hours, even days, and make it hard to do everyday activities. Several factors can trigger a migraine attack or increase a person’s risk of having an attack. These factors vary among individuals, including sudden weather or environment changes, too long or short sleep, strong smells or fumes, stress that causes changes in mental health and well-being, and too much physical stress on the body [ 2 ]. Climate refers to long-term weather patterns observed over large geographical regions. A combination of elements such as sunlight, temperature, pressure, wind, precipitation, cloud cover, and evaporation constitute the climate [ 4 ]. Another impact is climate change. The climate change induced global warming, and in particular the increased frequency and intensity of heat waves, have been linked to health problems. Among them, scientific works have been reporting an increased incidence of neurological diseases, encompassing also neurodegenerative ones. Therefore, it is very important for healthcare professionals and the general public to be aware of the potential health and environmental risks associated with climate change and to take necessary precautions [ 5 ]. Human behavior takes a pivotal part not only in driving global climate change but also in shaping the response to it [ 6 ]. Effective management of climate change requires high levels of public awareness and the implementation of transparent, accessible policies [ 7 ]. Türkiye has historically been one of the countries most severely influenced by climate change [ 8 ]. While global incidence studies report migraine rates of 1.5-2 per 1,000 in men and 3–6 per 1,000 in women, the rates in Türkiye are significantly higher because of both genetic and environmental factors, with reported rates of 2.98% in women and 1.93% in men [ 9 – 12 ]. Therefore, it is believed that climate-related changes may significantly affect the frequency and severity of headaches in individuals with migraine. Thus, this study was designed to address the effects of climate changes on headache symptoms among patients with migraine. The primary research question is as follows: Which climatic variables affect the headaches experienced by migraine patients? Methods This study is a descriptive research conducted to examine the impact of climatic factors on headaches experienced by individuals diagnosed with migraine. Participants The data collection was conducted through face-to-face surveys with migraine patients who presented to the neurology outpatient clinic of an educational and research hospital. Data collection took place between 1 October 2024 and 1 April 2025. Patients aged 18 to 63 years who consented to participate were included in this study. Post-hoc Power Analysis In the study, post-hoc power analysis (G*Power 3.1) was performed based on the relationship between migraine duration and pain score (r = 0.329), which was found to be statistically significant in correlation analyses. With a sample of 153 participants, it was determined that this relationship was detected with 99.5% statistical power (α = 0.05). Measures The data collection tools included the Personal Information Form, Climate Effects Form, Visual Analog Scale (VAS), and the Headache Impact Test (HIT-6). The Personal Information Form, developed by the researchers, comprised 13 questions regarding the sociodemographic characteristics of migraine patients. The Climate Effects Form consisted of 10 items and was developed by reviewing the literature, incorporating expert opinions from one assessment and evaluation specialist, one neurologist, and one nurse. Visual Analog Scale (VAS) It is used to transform certain non-quantifiable values into numerical data. It is a globally accepted, reliable, and easily applicable instrument [ 13 ]. Headache Impact Test (HIT-6) It evaluates domains such as vitality, pain, psychological distress, social functioning, role performance, and cognitive functioning. Among the six items, three specifically refer to the past four weeks, while the remaining three do not designate a specific timeframe. Items are scored using a 5-point Likert scale (6 = never, 8 = rarely, 10 = sometimes, 11 = very often, 13 = always). The total score ranges between 36 and 78, calculated by summing the scores of the six items. The level of impact is categorized as follows: ≤49 (little to no impact), 50–55 (some impact), 56–59 (substantial impact), and ≥ 60 (severe impact), with higher scores indicating greater impact [ 14 ]. Statistical analyses The data obtained in this study were analyzed using the SPSS 22.0 software. In the study, mean, standard deviation, minimum and maximum values for continuous variables and frequency (n) and percentage (%) distributions for categorical variables were calculated. Chi-square (χ²) test was used to evaluate the relationships between nominal and ordinal categorical variables. Analyses were supported by Monte Carlo simulation when more than 20% of the expected cell frequencies in subgroups were less than 5. For comparisons of ordinal data of three or more groups, the Kruskal-Wallis H test was used because the data did not meet the assumptions of parametric testing. For comparisons with significant results, two-way post-hoc analyses were performed to determine group differences (e.g. Bonferroni or Dunn-Bonferroni correction). The relationships between continuous or ordinal variables were analyzed with Spearman correlation coefficient. Correlation coefficients (r) and p values were used to evaluate the relationships between migraine duration, average attack duration, monthly attack frequency, headache impact score (HIT-6) and pain intensity. Correlation levels were interpreted as follows: 0.10–0.29 weak, 0.30–0.49 moderate and 0.50 and above strong. Significance level was set at p 0.05 were considered not significant. Results The mean age of the participating migraine patients was 37.10 ± 9.79 years, the average duration of migraine diagnosis was 7.93 ± 8.10 years, the average duration of a migraine attack was 11.73 ± 16.63 hours, and the mean number of migraine attacks per month was 3.39 ± 2.03. Furthermore, 83.7% of the patients were female, 32.0% had a bachelor’s degree, 75.8% were single, 77.1% had a moderate income level, and 56.2% were employed. Of the participants, 46.7% reported having at least one first-degree relative with migraine, only 9.2% had received any form of education regarding migraine, and 66.7% reported regular use of their prescribed medications (Table 1 ). In the study, it was determined that the pain scores of those who said that the increase in humidity “reduces” headache were significantly lower ( p 0.05) (Table 3 ). Only those who stated that “the effect of global warming on the seasons” “increases” headache had significantly higher HIT-6 total scores than those who stated that it ‘decreases’ or “does not affect” headache ( p < 0.05) (Table 4 ). Although the difference by gender was not significant, women more frequently reported temperature increases (72.7%) and wind (69.5%) as triggers, whereas men were more likely to report that climatic factors had no effect. It was also found that cold weather and increased humidity were more frequently reported as triggers by single individuals compared to married ones. Employed individuals were more likely to perceive climate as a triggering factor. In addition, the proportion of individuals who had received education on migraine and reported that seasonal transitions and rain had “no effect” was found to be higher. The study also found that among the groups regarding temperature increase, the diagnosis time was significantly lower in the group stating “reduces” (2.62 years). In other words, a weak, but approaching significance trend was observed between the perceptions regarding the effect of temperature and the duration of clinical experience. In terms of migraine diagnosis time, among the groups stating “cold weather increases, decreases or does not affect”, the average diagnosis time of the group stating “reduces” was determined to be higher (10.10 years) than the other groups. In terms of migraine diagnosis time, the average diagnosis time with the wind-related groups varied between 7 and 9 years in all three groups. Although the average time seemed slightly longer in those stating “reduces”, this difference was not significant. There is a statistically significant difference between the groups according to rain and drought in terms of the number of migraine attacks per month. It was found that the values were significantly higher in those who said “decreases”. In addition, there is a statistically significant difference between the groups regarding the effect of global warming on the seasons in terms of the average duration of migraine attacks. It was determined that the values were significantly lower in those who said “It does not affect”. Discussion The effects of climate change and air pollution on neurological health, disease progression, and clinical applications were documented in the literature. Specifically, extreme temperatures and climatic variability were linked to the incidence and severity of stroke, migraine headaches, hospitalizations among patients with dementia, and exacerbations of multiple sclerosis [ 15 ]. The classification of migraine triggers remains complex [ 16 ]. Potential triggers for migraine headaches include stress, hormonal changes during menstruation, ovulation, pregnancy, skipped meals, weather changes, excessive or insufficient sleep, exposure to odors (such as perfumes, colognes, and petroleum distillates), light exposure, alcohol consumption, smoking, and certain foods [ 1 , 17 ]. Given this context, the importance of studies on migraine and climate-related factors is evident. Therefore, the present study investigated the effects of climatic variables on migraine-related headaches. The findings revealed no significant differences in response to climatic variables or in the total HIT-6scores. There are similar studies in literature. For example, Toan et al. (2014) investigated perceptions of climate change and its health impacts among residents in Hanoi and reported that the most commonly experienced symptoms during hot weather were headache, fatigue, and dizziness [ 18 ]. In another study, Yang et al. (2011) examined data from a large-scale epidemiological study on headache incidence in the Greater Taipei area and identified significant correlations between headache occurrence and variables such as temperature, atmospheric pressure, duration of sunlight exposure, and relative humidity [ 19 ]. Elser et al. (2024) conducted a comprehensive review of peer-reviewed literature published between 2017 and 2022, focusing on environmental risk factors for primary headache disorders, which affect more than half the global population. Their findings associated high ambient temperatures and weather variability with headache occurrence [ 20 ]. In the present study, patients with migraine most frequently reported that their headaches were exacerbated by increased temperatures, seasonal transitions, and wind. Additionally, they noted that cold weather tended to alleviate their headaches, while rainfall had no noticeable effect. Similar findings have been reported in the literature. In a review conducted by Iba et al. (2023) examining migraine triggers across Asia, it was found that climate-related environmental homeostasis triggers reported by patients varied considerably between regions. These triggers included changes in weather, environmental fluctuations, cloudy conditions, cold and frigid temperatures, exposure to bright light, excessive environmental stimuli, heat, hot and humid climates, heat exposure, hot weather, humidity, sudden temperature shifts, summer season, sun/heat effects, climatic conditions, and wind [ 21 ]. Akgün et al. (2021) compared the effects of meteorological variables (including temperature, wind speed and direction, humidity, atmospheric pressure, UV index, and duration of sunlight) on the incidence, duration, and severity of episodic migraine (EM) and episodic tension-type headache (ETTH). Their findings indicated that the average wind speed during EM attacks was significantly higher than that observed during ETTH episodes [ 22 ]. Li et al. (2019) investigated the relationship between temperature, relative humidity, barometric pressure, and ambient air pollutants and the onset of migraine headaches among 98 patients with episodic migraine in the Greater Boston area of Massachusetts. They found that relative humidity and warmer seasons were associated with the onset of migraine attacks [ 23 ]. Hirata et al. (2023), using nationwide data from Japan, aimed to present current epidemiological data on the impact of primary headaches on daily activities, healthcare utilization, clinical characteristics, and pain severity/disability. They identified common triggers as fatigue for both migraine and tension-type headaches, while weather-related events and seasonal changes were found to be specific triggers for migraines. In the same study, weather-dependent factors and seasonal transitions (e.g., severe weather such as typhoons: 41.4%, seasonal changes: 30.1%, and sunny or rainy days: 27.6%) were commonly cited as migraine triggers [ 24 ]. Finally, Hoffman et al. (2015) investigated the potential correlation between specific meteorological variables and the frequency and intensity of migraine attacks, as well as the individual predictability of migraine episodes based on weather patterns and fluctuations. Their findings showed that only a subset of participants (approximately 13%) could be clearly identified as weather-sensitive with respect to migraine events [ 25 ]. Similarly, Denney et al. (2025) reviewed the most recent research on how weather may influence migraine and concluded that migraine attacks could be associated with climatic variables such as barometric pressure, humidity, and wind [ 26 ]. In this study, when the effects of climatic factors on headaches experienced by patients with migraine were analyzed in terms of demographic variables, no significant differences were found regarding gender, educational attainment, income level, employment status, or prior education about migraine. Similarly, in the literature, a study carried out by Tanik et al. (2020), which aimed to investigate the relationship between exposure to hot/cold weather and the characteristic clinical features of headaches in patients with migraine and tension-type headaches, reported no significant relationship between exposure to temperature extremes and age, BMI, or the clinical characteristics of headaches in migraine patients [ 27 ]. Moreover, in a study carried out by Akgün et al. (2021), it was determined that as the mean age of migraine patients increased, the average temperature on days with migraine attacks tended to decrease [ 22 ]. These results suggest that migraine, characterized by episodes of severe headache, can indeed be influenced by climatic changes. Migraine, which manifests as headache episodes triggered by various factors, can significantly impair patients’ quality of life. Among the key triggers, climate changes are known to have adverse effects on human health. Climate change can contribute to mortality, infectious diseases, and related respiratory illnesses. Therefore, it is very important for healthcare professionals to be informed about the potential effects of climate change on both acute and chronic health conditions. In this study, it was observed that migraine patients reported being particularly affected by increases in temperature, seasonal transitions, and wind. Although the associations were not significant, some sociodemographic variables were also observed to have potential effects. Given these findings, it is recommended that further studies be carried out focusing on climate-related variables among specific subgroups, particularly unmarried individuals, employed individuals, duration of migraine, average duration of migraine attack, or number of migraine attacks those who have not received education on migraine. Moreover, targeted informational efforts and patient education programs may be developed to address the influence of climatic factors, especially cold weather and humidity, on migraine and to enhance patients’ coping strategies. The data collection tools included a Personal Information Form, Climate Effects Form, Visual Analog Scale (VAS), and Headache Impact Test (HIT). The sample consisted of 153 migraine patients.\u003c/p\u003e\u003cp\u003e\u003cb\u003eResults\u003c/b\u003e\u003c/p\u003e\u003cp\u003eThe participants had a mean age of 37.10\u0026thinsp;\u0026plusmn;\u0026thinsp;9.79 years, and the average duration of migraine diagnosis was 8.17\u0026thinsp;\u0026plusmn;\u0026thinsp;8.15 years. Of the participants, 83.7% were female, 32.0% had a bachelor\u0026rsquo;s degree, 75.8% were single, 77.1% had a middle income level, and 56.2% were employed. In the study, participants most frequently reported that increases in temperature (71.9%), seasonal transitions (71.2%), and wind (68.0%) exacerbated their headaches.
