Radiographers’ adherence to radiation protection practices can help reduce such risks. The study assessed radiographers’ knowledge and practices, identified barriers to adherence and explored ways to enhance radiation safety practices in Northern Ghana. Methods A cross-sectional study using consecutive sampling was conducted among 38 radiographers in Northern Ghana. Data were collected using a self-administered semi-structured questionnaire and analysed using IBM SPSS (version 25). Descriptive statistics were presented as means and standard deviations for continuous data and frequencies and percentages for categorical data. Group differences were assessed using independent sample t-test and one-way ANOVA with statistical significance set at P < 0.05. Results Most participants (n = 19, 63.2%) received radiation safety training within the past three years. The mean personnel radiation protection score was high (13.47 ± 1.64). However, knowledge score on ALARA and deterministic/stochastic effects concept was high (13.47 ± 1.64), misconceptions about radiation effects persisted. A statistically significant relationship between years of experience and adherence to environmental protection practices (p = 0.002) was found. Although most radiographers (n = 37, 97.4%) had radiation protection equipment, (n = 7, 18.4%) reported not using the equipment. Conclusion Radiographers had satisfactory knowledge regarding radiation protection, but adherence was inconsistent. Equipment deficiencies hampered radiation safety compliance and practical knowledge application. A multifaceted approach is required to eliminate the knowledge-practice gap, including frequent practical training, supervision and providing equipment to aid compliance. Radiation protection ionising radiation radiographers radiation safety culture radiation safety compliance INTRODUCTION Radiation protection involves safeguarding radiographers, patients, and the environment from the harmful effects of ionising radiation exposure while ensuring that radiation doses to patients are commensurate with the intended medical purpose (IAEA., 2018; IAEA, 2020 ). Ionising radiation is used in many diagnostic radiography modalities such as conventional radiography, computed tomography (CT), and interventional fluoroscopy. Globally, an estimated 250 million diagnostic radiological exams are conducted annually (Lakhwani et al., 2019 ). Although diagnostic radiography is essential to patient care, the use of ionising radiation presents potential health risks to patients, practitioners, and the public (Fiagbedzi et al., 2022 ).These risks include cataracts, infertility, cancer, dyscrasia, genetically determined ill-health, and degenerative diseases (Abuzaid et al., 2019). Importantly, research has identified that these effects result from overexposure often due to insufficient protection (NG & SA, 2020). The demand for medical exposures for diagnostic purposes has increased, making medical radiation exposure the largest source of average annual manmade radiation exposure (Hricak et al., 2011 ). Given the dynamic nature of radiation protection practices, radiographers need to stay updated with current trends And adhere to safe work practices and radiation protection protocols in order to mitigate radiation exposure hazards. The current population of Ghana stands at approximately 32 million (Ghana Statistical Service, 2021 ), with a total of 674 diagnostic imaging equipment across 266 medical imaging facilities including general X-ray, dental X-ray, fluoroscopy, mammography, CT, MRI, and SPECT machines (Bour et al., 2022 ). The Northern region, contributing 3% of the total diagnostic imaging units in Ghana has 17 imaging units and 9 diagnostic medical centres mostly concentrated in Tamale with the Tamale Teaching Hospital housing the majority of equipment and radiographers. The commonly used modalities in the region are general X-ray and CT (Bour et al., 2022 ) Knowledge of radiation protection is vital for radiographers to regulate exposures to patients, the environment, and themselves, recognising that even minimal doses can be harmful (Maharjan et al., 2020 ). Adequate knowledge and availability of radiation protection equipment and apparels are essential for adherence to safety practices during examinations. Studies in the Central Region of Ghana indicated high knowledge of radiation protection among radiographers, but low adherence levels (Fiagbedzi et al., 2022 ). This study builds on the previous work by Fiagbedzi et al. ( 2022 ) to (i) evaluate the knowledge level and radiation protection practices and (ii) identify barriers to radiation protection adherence and (iii) ways to improve radiation safety practices. MATERIAL AND METHODS Study Site The study was conducted in healthcare facilities with radiology units situated within the five regions in Northern Ghana. This allowed for investigation of variations in adherence to radiation safety requirements across different healthcare settings and ensured the inclusion of diverse circumstances. Study Design and Population The study employed a cross-sectional study design to assess the knowledge level and adherence to radiation protection practices among registered radiographers operating within health facilities with radiology units, both public and private imaging centres in Northern Ghana. Radiographers registered under the Allied Health Professions Council (AHPC) and the Ghana Society of Radiographers (GSR) were recruited as participants. Radiographers working in general X-ray, computed tomography (CT), and mammography units were recruited for the study. Sample Size and Sampling Procedure Consecutive sampling was used to include all thirty-eight radiographers from the five northern regions of Ghana to participate in this study. Inclusion and Exclusion Criteria Participants with at least a national diploma or a Bachelor of Science degree in Diagnostic Radiography and registered under the AHPC and GSR were recruited. Radiographers registered under these bodies without active pins, senior radiographers occupying administrative positions and student radiographers on clinical placements were excluded. Data collection A self-structured questionnaire was developed to assess the knowledge level and adherence of radiographers after reviewing literature. A pilot study was conducted with four representatives from the participating radiographers to test reliability and validity. Responses gathered from the pilot were consistent with the study objectives confirming the reliability of the questionnaire. The final version of the questionnaire was distributed electronically using Google Forms to facilitate wide and convenient access. The self-administered questionnaire comprised four sections; section A: Social demographics (age, academic qualifications, and work experience), B: Current radiation protection practices whilst adherence to radiation protection protocols was assessed using a four-point Likert scale, C: Knowledge level in radiation protection and D: Availability of radiation protection devices in various radiology units. Data Analysis Statistical Package for Social Sciences (SPSS) (version 25) was used to analyse the data. Frequency distribution, pie charts, and bar charts were used to present the responses. Descriptive statistics were presented as means and standard deviations for continuous data and frequencies and percentages for categorical data. Differences between groups were assessed using independent sample t-test and one-way ANOVA. A significance level of p < α; α = 0.05 was used to determine statistical significance. Ethical Consideration Permission and ethical approval were obtained from the Ghana Society of Radiographers (GSR/NEC/GS/RS-09/06/24) and the University for Development Studies Institutional Review Board (UDS/RB/121/24). Implied informed consent was obtained from participants and the right to withdraw from the study at any point was clearly communicated to all participants. RESULTS Socio-Demographic Characteristics of Participants Majority of participants were males (n = 31, 81.6%) with almost half (n = 13, 47.3%) between the ages of 20–30. In terms of education and training, majority (n = 23, 60.5%) had a bachelor's degree and, similarly, most participants (n = 31, 81.6%) had undergone training in radiation safety, of which 63.3% (n = 19) had their training within 1–4 years ago. The socio-demographic characteristics of the participants are provided in Table 1 . However, only 20.0% (n = 7) consistently wore the badges. Similarly, only 23.6% (n = 9) of participants wore lead gloves, goggles and thyroid collar during theatre radiography. Table 2 presents results on the radiation protection measures and practices reported by participants. All participants (n = 38) indicated awareness of standard radiation protection principles, including ALARA (As Low As Reasonably Achievable), the relevance of time, distance and shielding (IAEA., 2018; ICRP, 2007). However, findings revealed some misconceptions. Notably, 92.1% of participants identified hair loss as a common effect of radiation but in reality, such effects occurs at higher doses in therapeutic radiography (Vickers et al., 2021 ; Zawrzykraj et al., 2023 ). The threshold radiation doses for deterministic effects of hair loss by the International Commission on Radiological Protection (ICRP) are 3–6 Gy for temporary hair loss and 7 Gy for permanent hair loss whilst diagnostic radiography doses are in milligray(mGy) (Nwokorie et al., 2011 ; Smith-Bindman et al., 2015 ; Ounsakul et al., 2016 ). Discrepancies in response about the use and purpose of TLD badges emphasised high theoretical knowledge but limited operational understanding of these RP principles (Fiagbedzi et al., 2022 ). The training status on RP is significant; despite 63.2% of radiographers in Northern Ghana having received RP training in the last three years, there was no statistically significant difference between knowledge scores and training status (p = 0.178), indicating that training maybe adequate but no regular reinforcement and application in their practice (Fiagbedzi et al., 2022 ; Maharjan et al., 2020 ). Foundational knowledge among radiographers in Northern Ghana is adequate; nonetheless there should be interventions aimed to correct radiographers misconceived perception about RP and reinforce operational competency to safeguard and promote standard RP safety practices in various radiology units. Adherence to Standard Radiation Protection Practices Evaluating adherence to standard RP practices revealed a significant knowledge gap and practice. All radiographers reported adherence to collimation, correct source to image distance and use of anatomical markers but lapses were observed in personnel and patient protection practices. Though 86.8% of radiographers had TLD badges, only 20% wore them consistently during working hours. This is similar to results from previous studies (Eze et al., 2013 ; Fiagbedzi et al., 2022 ; Abuzaid et al.,2019 ) where 25%, 50% and 63% of radiographers wore their TLD badges all the time respectively. 55.3% of radiographers do not wear lead aprons routinely despite its availability in most facilities and only 23.6% consistently used lead gloves, thyroid collars and lead goggles during theatre radiography. Similarly, while a high percentage of radiographers adhered to proper collimation and proper source to image distance, only 60.5% reported using gonad shields and 47.4% repeated exposures sometimes. Higher percentage of radiographers enquire about pregnancy status from females of reproductive age. This is essential to avoid carcinogenic and mutational effects of radiation to the foetus (Fiagbedzi et al., 2022 ; Kaplan et al., 2018 ). Furthermore, participants demonstrated good adherence to environmental protection practices. Radiographers provided lead aprons for comforters and ensured doors are closed before exposure. Notably, 76.3% had functional radiation warning lights, highlighting the need for further improvement. This pattern indicates an issue observed in other low- and middle-income nations, where practice fails to meet requirement despite high knowledge levels (Maina et al., 2020 ). The lack of notable correlations between adherence and demographic variables indicates that institutional policies and the work environment have a greater effect on radiation protection practices than individual factors (Fiagbedzi et al., 2022 ; Maharjan et al., 2020 ). Availability and Utilization of Radiation Protection Equipment Radiographers’ ability to adhere to safety principles and practices were affected by variations in the availability of the radiation protection equipment. Lead aprons were widely available (100%) across all facilities; however, other essential protective equipment were noticeably few, including lead thyroid collars (28.9%), lead gloves (34.2%), lead goggles (23.7%), breast shield (5.3%) and gonad shields (50%). This is similar to results from Maina et al. ( 2020 ) study in Rwanda where 99.13% of lead rubber aprons and 97.4% of lead equivalent barriers were the most available RP. They reported that during installation, vendors should provide radiological equipment with appropriate radiation safety accessories, instead of giving them as alternative accessories. Effective radiation protection practices jeopardised by unavailability of equipment particularly for the sensitive anatomical parts such as eyes, gonads, thyroid gland and breast. Comparably, only 39.5% of facilities had immobilisation devices affecting measures to minimize repeat exposures. Utilization patterns reflect challenges linked with availability. 81.6% of radiographers used protective devices while 18.4% did not, citing challenges such as patient non-compliance, device discomfort and unavailability of devices. The strong relationship shown between the availability of protective equipment and compliance with safety procedures emphasize how important it is to equip radiology facilities to enforce strict safety regulations (ICRP, 2007; IAEA, 2020 ). Study Limitations This study could be bias due to self-reported practices where participants may have overstated their adherence. We therefore suggest future studies on this topic by direct observation of participants instead of self-reported practices by participants. Certain facilities had unqualified personnel acting as radiographers and some qualified radiographers failed to participate limiting the comprehensiveness of the data. Higher-dose modalities such as computed tomography and fluoroscopy were lacking in majority of the facilities restricting the evaluation of protection practices for higher-dose modalities. CONCLUSION Radiographers in Northern Ghana exhibited adequate theoretical understanding of radiation protection principles. However, their adherence to standard RP practices is inconsistent, significantly affected by the unavailability of protective equipment and institutional circumstances. A multifaceted approach is required to close the knowledge-practice gap including frequent practical training to radiographers in Northern Ghana by the Radiation Protection Institute of the Ghana Atomic Energy Commission. Extensive radiation protection equipment provision, rigorous operational enforcement, and a more general cultural commitment to radiation safety in all healthcare facilities should be a collective agenda for all radiology units. Lead aprons were widely available (100%) across all facilities; however, other essential protective equipment were noticeably few, including lead thyroid collars (28.9%), lead gloves (34.2%), lead goggles (23.7%), breast shield (5.3%) and gonad shields (50%). This is similar to results from Maina et al. (\u003cspan citationid=\"CR13\" class=\"CitationRef\"\u003e2020\u003c/span\u003e) study in Rwanda where 99.13% of lead rubber aprons and 97.4% of lead equivalent barriers were the most available RP. They reported that during installation, vendors should provide radiological equipment with appropriate radiation safety accessories, instead of giving them as alternative accessories. Effective radiation protection practices jeopardised by unavailability of equipment particularly for the sensitive anatomical parts such as eyes, gonads, thyroid gland and breast. Comparably, only 39.5% of facilities had immobilisation devices affecting measures to minimize repeat exposures. Utilization patterns reflect challenges linked with availability. 81.6% of radiographers used protective devices while 18.4% did not, citing challenges such as patient non-compliance, device discomfort and unavailability of devices.
